Neoadjuvant chemotherapy plus intensity-modulated radiotherapy versus concurrent chemoradiotherapy plus adjuvant chemotherapy for the treatment of locoregionally advanced nasopharyngeal carcinoma: a retrospective controlled study

Abstract: BackgroundIn the era of intensity-modulated radiotherapy (IMRT), the role of neoadjuvant chemotherapy (NAC) for locoregionally advanced nasopharyngeal carcinoma (NPC) is under-evaluated. The aim of this study was to compare the efficacy of NAC plus IMRT and concurrent chemoradiotherapy (CCRT) plus adjuvant chemotherapy (AC) on locoregionally advanced NPC.MethodsBetween January 2004 and December 2008, 240 cases of locoregionally advanced NPC confirmed by pathologic assessment in Sun Yat-sen University Cancer Ce… Show more

“…The 5-year LRFS, DMFS, PFS, and OS rates for T4 stage were 87.2%, 65.8%, 59.4%, and 62%, respectively. The results of our study are generally comparable to other studies 1820. In our observational cohort study, 20 patients experienced locoregional failure and total distant metastasis occurred in 51 patients.…”

“…Owing to its pathological features and the radiosensitivity of NPC, radiotherapy has been a major modality in the treatment of NPC [3] . Concurrent chemoradiotherapy (CCRT) either with or without induction (neoadjuvant) chemotherapy is more beneficial than radiotherapy alone for the treatment of advanced NPC patients [4][5][6] . There is little doubt that chemoradiotherapy improves treatment outcomes and reduces recurrence and death rates.…”

“…Concurrent chemotherapy with IMXT is the current standard treatment for locally advanced NPC and has been studied against other chemotherapy scheduling [18, 19]. However, the addition of chemotherapy during IMXT for re-irradiation of locally recurrent NPC has never been studied in any prospective clinical trial.…”
