Needleless connector decontamination for prevention of central venous access device infection: A pilot randomized controlled trial

Abstract: Pilot randomized controlled trial (180 patients) of needleless connector decontamination. Central line-associated bloodstream infection occurred in 2% (1/61) of 70% isopropyl alcohol (IPA) wipe, 2% (1/59) of 70% IPA cap, and zero (0/58) infections in 2% chlorhexidine gluconate in 70% IPA wipe patients. Larger definitive trials are feasible and needed.

“…Las principales limitaciones para el uso de la HDC son la potencial aparición de resistencias a la clorhexidina y el incremento de bacteriemia por gramnegativos, así como la tolerancia y los efectos adversos en los tratamientos prolongados 52 . Uso de tapones con solución antiséptica en los conectores 53 , 54 , 55 , 56 , 57 , 58 , 59 , 60 . (Evidencia moderada/ recomendación fuerte) …”

“…Only three studies reported safety data. 5,32,33 Two studies reported the use of Curos caps and did not observe damage to the access valves nor any other device-related adverse events. 5,32 One study investigating SwabCap reported that two needleless connectors became opaque and additionally observed 70% IPA seeping between the rubber inner and outer plastic.…”

“…However, this did not have any impact on the patient. 33 Costs Six studies evaluated costs of ABCs and reported significant annual cost savings. 11,14,16,18,28,29 Calculated costs per 1,000 catheter days range from $10,000 to $39,000, with one outlier of $130,144 per 1,000 catheter days (Table 2).…”
