Needle track seeding after percutaneous radiofrequency ablation of hepatocellular carcinoma: 14-year experience at a single centre

Abstract: Needle track seeding is a rare delayed complication after percutaneous RFA. Tumour >3 cm, subcapsular tumour, biopsy before RFA and non-cool-tip electrode are potential risk factors for seeding. Local therapies are effective methods for seeding patients.

“…Several authors suggested an increased risk of neoplastic seeding in HCC when ablation techniques were combined with biopsy and recommended to avoid biopsy whenever possible. Increased risk of seeding was also reported when Radiofrequency ablation (RFA) followed biopsy, even when track ablation has been performed [7][8][9]. The time interval between the biopsy and the discovery of the neoplastic seeding also varies greatly due to the quality and duration of patient follow-up.…”

“…The time interval between the biopsy and the discovery of the neoplastic seeding also varies greatly due to the quality and duration of patient follow-up. Treatment of choice for needle track seeding is radical surgical excision, local radiotherapy, or highintensity-focused ultrasound [8].…”

“…Methods of obtaining histological tissue from the liver such as liver biopsy and EUS-FNA carry a risk of needle-tract seeding in patients with malignant tumors [12,13]. In the present case, the mass in the left lobe of the liver resembled a metastasis from a GB cancer on CT at the time of the first surgery.…”

“…[10][11][12] Recently, risk of seeding after percutaneous ethanol injection (PEI), radiofrequency ablation (RFA), microwave ablation (MWA) and cryoablation varied from 0.3% to 1.9%, from 0.005% to 12.5%, from 0.44% to 0.75% and 0.76% in previous reports respectively, has received increased attention from clinicians. [13][14][15][16][17][18] Based on appropriate choice by the patient and careful attention to the ablation procedure, several studies showed low seeding rate after ablation. Nevertheless, even if the ablation modalities have been proven to be a relatively safe technique, seeding remains an inevitable ablation-related complication.…”
