In the light of the success and the expected growth of its arsenal, immuno-therapy may become the standard neoadjuvant procedure for many cancers in the near future. However, aspects such as the identity, organization and the activation status of the peri- and intra-tumoral immune cells would represent important elements to weigh in the decision for the appropriate treatment. While important progress in non-invasive imaging of immune cells has been made over the last decades, it falls yet short of entering the clinics, let alone becoming a standard procedure. Here, we provide an overview of the different intra-vital imaging approaches in the clinics and in pre-clinical settings and discuss their benefits and drawbacks for assessing the activity of the immune system, globally and on a cellular level. Stimulated by further research, the future is likely to see many technological advances both on signal detection and emission as well as image specificity and resolution to tackle current hurdles. We anticipate that the ability to precisely determine an immune stage of cancer will capture the attention of the oncologist and will create a change in paradigm for cancer therapy.

El diagnóstico y monitoreo de enfermedades pulmonares depende en gran medida de los estudios de imagen, las pruebas de función fisiológica y la toma de muestras de tejido. La tomografía de coherencia óptica (TCO) y la endomicroscopía confocal láser (ECL) son técnicas de imagen novedosas con resolución casi microscópica, que pueden combinarse de manera fácil y segura con la broncoscopía convencional. Mediante estas técnicas pueden visualizarse compartimentos anatómicos pulmonares relacionados con enfermedades, en tiempo real. En enfermedades pulmonares obstructivas, pueden identificarse y cuantificarse las capas en las paredes de las vías respiratorias y el remodelado estructural asociado. En neoplasias pulmonares, es posible discriminar entre áreas normales y malignas en las vías respiratorias centrales, el parénquima pulmonar, los nodos linfáticos y la pleura. Un número creciente de enfermedades pulmonares intersticiales (EPIs) han podido visualizarse mediante TCO o ECL. Es posible estudiar por imagen varios cambios estructurales asociados con EPIs: fibrosis, infiltración celular, bronqui(ol)ectasias, quistes y el patrón microscópico en panal de abejas. Aunque aún no se han implantado en la práctica clínica, la TCO y la ECL tienen el potencial de mejorar la detección y el monitoreo de enfermedades pulmonares y pueden contribuir a desentrañar la fisiopatología de varias enfermedades y el mecanismo de acción de nuevos tratamientos. De hecho, el examen de las paredes de las vías respiratorias mediante TCO podría ser útil al evaluar tratamientos contra el remodelado de las vías respiratorias. Al visualizar células malignas individuales, la ECL tiene potencial como herramienta para detectar el cáncer en tiempo real. En el futuro, ambas técnicas podrían combinarse con la detección de marcadores fluorescentes con láser. Esta revisión discute el valor de la TCO y la ECL en la medicina pulmonar, presentando de forma sumaria la evidencia actual, y se comentan las perspectivas a futuro.

Multimodal optical coherent tomography grows popularity with researchers and clinicians over the past decade. One of the modalities is lymphangiography, which allows visualization of the lymphatic vessel networks within optical coherence tomography (OCT) imaging volume. In the present study, it is shown that lymphatic vessel visualization obtained from the depth-resolved attenuation coefficient distributions, corrected for the noise, shows improved contrast and detail in comparison with previously proposed approaches. We also argue that the two most popular approaches for lymphatic vessel visualization, namely simple intensity thresholding and vesselness calculation based on local Hessian matrix eigenvalues, imply different definitions of the lymphatic vessel's appearance in the OCT volume and lead to the different networks.

Abstract: Our results demonstrate that OCT provides critical microstructural information that may be useful to guide TBNA lymph node sampling, as a complement to endobronchial ultrasound. In vivo studies are needed to further evaluate the clinical utility of OCT in thoracic lymph node assessment.
