Needle-based Optical Coherence Tomography to Guide Transbronchial Lymph Node Biopsy

Abstract: Our results demonstrate that OCT provides critical microstructural information that may be useful to guide TBNA lymph node sampling, as a complement to endobronchial ultrasound. In vivo studies are needed to further evaluate the clinical utility of OCT in thoracic lymph node assessment.

“…A means to overcome this constraint is to minimize the imaging probe to the size of a needle allowing tissue-imaging biopsies. Thus, internal human nodal B cell follicles and germinal centers can be observed (39) making it in theory possible to detect intra-tumoral tertiary lymphoid structures. Because of lack of molecular sensitivity, there have been efforts to combine OCT with fluorescence detection systems (40).…”

“…El mismo grupo de investigación estudió el uso de TCO para identificar metástasis en los nódulos linfáticos. Se practicó TCO con aguja en 26 nódulos linfáticos ex vivo, tanto sanos como con enfermedad metastásica [57]. Las características de la enfermedad metastásica fueron distintas que las de nódulos linfáticos sin cáncer, e incluso mostraron rasgos relacionados con el subtipo de tumor.…”

“…Optical coherence tomography (OCT) is an emerging tool for non-invasive and low-invasive visualization of the biological tissues with resolution in the order of several micrometers and penetration depth up to 2 mm [1][2][3][4]. Due to its properties, it became a popular tool for diagnostics [5][6][7][8], biopsy guidance [9][10][11], and positive surgical margins detection [12][13][14][15][16]. One of the trends in the OCT development which gains popularity is multimodality, that is, the ability to extract various information about the object from the same OCT dataset (or at least from the data collected with the same OCT device).…”
