Objective Necrotizing sialometaplasia (NS) is an uncommon benign process which affects minor and, more rarely, major salivary glands. While self‐limiting, the condition might be clinically and histologically mistaken for malignancy. Furthermore, NS may accompany neoplasms. The aim of this paper was to report a series of Italian patients affected by NS associated with an unusual high presence of neoplasms of minor and major salivary glands. Study Design Clinical and histological features of twelve patients with NS were retrospectively evaluated. Results Eight patients presented NS of the minor salivary glands of the palate, and two of them had associated neoplasms (pleomorphic adenoma and adenoid cystic carcinoma) at the same site. Four patients had NS of the parotid gland associated with a history of fine‐needle aspiration biopsy performed to diagnose parotid neoplasms. These were epithelial–myoepithelial carcinoma, adenoid cystic carcinoma, Warthin's tumor, and oncocytoma, respectively. Conclusion Tumors of minor and major salivary glands might be associated with NS more frequently than previously reported. While NS of the parotid is generally the result of invasive diagnostic procedure and is detected after the excision of the tumor, NS of the minor salivary glands may obscure an underlying neoplasm, resulting in delays in referral or treatment.

Introdução: A sialometaplasia necrosante (SN) é uma patologia rara, resultante de isquemia local do tecido da glândula salivar. Objetivo: Determinar a prevalência global da SN em humanos, e os fatores de risco desta patologia. Método: A estratégia de pesquisa foi de caráter descritivo, com o levantamento de dados eletrônicos nacionais e internacionais nas bases de dados Pubmed, Lilacs, Scopus, Web of Science e Google acadêmico. Foram encontrados 1839 artigos, finalizando em 37 artigos entre 2000-2020. Resultados: 46 casos: Palato 35 (74,4%), lábio inferior 5 (10,6%), glândula parótida 3 (6,5%), vestíbulo bucal 1 (2,1%), cavidade nasal 1 (2,1%), hipofaringe 1 (2,1%).13 por cirurgias anteriores; 6 sem queixas precedentes; 3 apontaram distúrbios alimentares; 3 reportaram uso de medicações spray, os demais apresentaram vários fatores envolvidos. Conclusão: A SN tem maior prevalência no sexo masculino de meia idade, acometendo mais o palato após cirurgias, sendo este considerado o principal fator de risco.

Necrotizing sialometaplasia is a benign lesion affecting the minor salivary glands of the hard palate. This lesion may be clinically and histopathologically confused with malignant lesions. A case of a 47-year-old man who presented with necrotizing sialometaplasia on the left side of the hard palate is herein reported. A biopsy was performed, and the condition was diagnosed based on immunohistochemistry. The lesion receded following treatment with tranexamic acid and sodium azulene sulfonate. The symptom of painful swelling on the hard palate subsided within 10 days. The palatal lesion had disappeared completely 4 months later.
