Natural History of Progressive Adult Scoliosis

Abstract: The originality of our study is the diagram. We demonstrated that the rate of progression was linear, and it can be used to establish an individual prognosis. The diagrams visualized 2 main distinct types. There was a significantly faster rate of progression in type B. In type A, rotary subluxation occurs during progression of the curvature. In type B, it seems to be the initial event. Menopause is a period of deterioration in type B.

“…In cases of progressive adolescent idiopathic scoliosis, the curvatures continue to increase 1˚ -1.5˚ yearly in adulthood at a nearly linear rate [7]. It would appear that all 3 patients have altered the natural course of their curvatures.…”

“…Based on the natural history of scoliosis [7], it is unlikely that the observed improvements in these patients were attributed to natural history, but rather due to the prescribed treatment.…”

“…De novo adult scoliosis is most commonly due to asymmetric disc degeneration and facet joint degeneration. 10 The latter forms a vicious cycle: the progression of lumbar curvature results in increasing joint stress, and increasing joint stress results in more curvature, thus furthering scoliotic disease. The most common site of the apex of curvature is the L2/3 and L3/4 joint, but most adults will have additional thoracolumbar curvature in addition to lumbar curvature.…”

“…Krivina se navadno počasi povečuje tudi v odraslosti. Linearno povečevanje za približno eno Cobbovo stopinjo na leto je bilo ugotovljeno pri napredujoči skoliozi odraslih (22). Skolioza se lahko v odraslosti, ko lahko zanjo krivimo le kostno preoblikovanje, pojavi tudi na novo.…”
