One of the current challenges in wound care is the development of multifunctional dressings that can both protect the wound from external agents and promote the regeneration of the new tissue. Here, we show the combined use of two naturally derived compounds, sodium alginate and lavender essential oil, for the production of bioactive nanofibrous dressings by electrospinning, and their efficacy for the treatment of skin burns induced by midrange ultraviolet radiation (UVB). We demonstrate that the engineered dressings reduce the risk of microbial infection of the burn, since they stop the growth of Staphylococcus aureus. Furthermore, they are able to control and reduce the inflammatory response that is induced in human foreskin fibroblasts by lipopolysaccharides, and in rodents by UVB exposure. In particular, we report a remarkable reduction of pro-inflammatory cytokines when fibroblasts or animals are treated with the alginate-based nanofibers. The down-regulation of cytokines production and the absence of erythema on the skin of the treated animals confirm that the here described dressings are promising as advanced biomedical devices for burn management.

Polycaprolactone (PCL) is a non-toxic and slowly biodegradable synthetic polymer, and is used in making biomaterial products such as surgical sutures, scaffolds, fibres and textile mesh for biomedical applications. Compared to materials of biological origin, PCL has no intrinsic biological capability to enhance healing of damaged cells or tissues. Therefore, a variety of bioactive agents such as proteins are incorporated into polymer matrices to improve biological interactions between the implanted device and cells or tissues. However, addition of structurally different proteins can greatly influence the key physicochemical characteristics of biomaterials, such as mechanical performance and surface morphology. In the present study, keratin extracted from wool was added to the PCL matrix and fibres were extruded using a melt-extrusion technique. Pure PCL fibre had an average tensile strength of 981 MPa, elongation of 19% and Young's modulus of 5.8 GPa. The addition of keratin into the PCL matrix, imposed melt-processing difficulty and reduced tensile strength at higher loadings of keratin (e.g., 10 wt%). However, silane treatment improved the keratin/PCL interfacial adhesion as revealed by electron microscopy and consequently improved melt-spinning, and reduced mechanical stiffness of the material, a key characteristic of suture fibres.

Wound healing occurs by a series of interrelated molecular events which work together to restore tissue integrity and cellular function. These physiological events occur smoothly in normal healthy individual and/or under normal conditions. However, in certain cases, these molecular events are retarded resulting in hard-to-heal or chronic wounds arising from several factors such as poor venous return, underlying physiological or metabolic conditions such as diabetes as well as external factors such as poor nutrition. In most cases, such wounds are infected and infection also presents as another complicating phenomenon which triggers inlammatory reactions, therefore delaying wound healing. There has therefore been recent interests and signiicant eforts in preventing and actively treating wound infections by directly targeting infection causative agents through direct application of antimicrobial agents either alone or loaded into dressings (medicated). These have the advantage of overcoming challenges such as poor circulation in diabetic and leg ulcers when administered systemically and also require lower amounts to be applied compared to that required via oral or iv administration. This chapter will review and evaluate various antimicrobial agents used to target infected wounds, the means of delivery, and current state of the art, including commercially available dressings. Data sources will include mainly peer-reviewed literature, clinical trials and reports, patents as well as government reports where available.
