Defective Natriuretic Peptide Receptor Signaling in Skeletal Muscle Links Obesity to Type 2 Diabetes

Circulating natriuretic peptide (NP) levels are reduced in obesity and predict the risk of type 2 diabetes (T2D). Since skeletal muscle was recently shown as a key target tissue of NP, we aimed to investigate muscle NP receptor (NPR) expression in the context of obesity and T2D. Muscle NPRA correlated positively with wholebody insulin sensitivity in humans and was strikingly downregulated in obese subjects and recovered in response to diet-induced weight loss. In addition, muscle NP clearance receptor (NPRC) increased in individuals with impaired glucose tolerance and T2D. Similar results were found in obese diabetic mice. Although no acute effect of brain NP (BNP) on insulin sensitivity was observed in lean mice, chronic BNP infusion improved blood glucose control and insulin sensitivity in skeletal muscle of obese and diabetic mice. This occurred in parallel with a reduced lipotoxic pressure in skeletal muscle due to an upregulation of lipid oxidative capacity. In addition, chronic NP treatment in human primary myotubes increased lipid oxidation in a PGC1a-dependent manner and reduced palmitate-induced lipotoxicity. Collectively, our data show that activation of NPRA signaling in skeletal muscle is important for the maintenance of long-term insulin sensitivity and has the potential to treat obesity-related metabolic disorders.

ABSTRACT:The prevalence of overweight and obesity have increased dramatically worldwide and together they constitute a major risk factor for coronary heart disease. The aim of this study was to assess the effect of acute high intensity aerobic training on ANP and Endothelin-1 in inactive obese women. In order to nineteen obese women mean age± SD: 27.94± 3.30, mean weight ±SD: 88.13 ±7.28, mean height ±SD: 163.00± 4.91, mean BMI ±SD: 32.96± 3.13 selected and were randomly allocated to experimental and control groups. Experimental group performed a session acute aerobic exercise on Ergometer at intensity 25w that increased every two minutes 25w to workload and performed to exhaustion every subject.Samples blood was taken after 12 hours fasting, before and after of program training. For analyzed of biochemical variables used ELISA method and for analyses data used ANOVA. The results of this study showed that acute aerobic training causes significant increase in the level of plasma ANP in obese women (p=0. 006). But no significant differences observe in plasma level of Endothelin-1. Finally, on the results of this study, one session acute aerobic training causes significant increase in level ANP and may represent an optimal therapeutic agent in cardiovascular diseases.Also the NP pathway may offer a novel therapeutic avenue for the management of obesity and type 2 diabetes.

Type 2 diabetes mellitus (T2DM) is one of the most prevalent metabolic diseases that associated with increased risk for cardiovascular (CV) diseases and newly CV events. Although CV risk assessment is incorporated in primary and secondary prevention strategies to improve morbidity and mortality that are applied in diabetic patient, it is important to stratify individuals at high CV risk not just prior diabetic complication, but at early stages of development of the CV diseases. The aim of the editorial comment is to discuss possible predictive role of cardiac biomarkers in T2DM. CV biomarkers may contribute to improved prediction of mortality and CV events in T2DM. It has suggested that measurement of serum levels of hs-CRP, galectin-3, natriuretic peptides, fibroblast growth factor-23, α-klotho, and hs-cTnT probably allows the screening of diabetes patients at risk of CV events. Future directions are associated with discovering of novel biomarkers and optimal combinations of recently used markers to provide additional prognostic information beyond what is available with other traditional CV risk factors.
