Nationwide survey of pediatric gastrointestinal endoscopy in Japan

Abstract: Background and Aim The implementation rates of pediatric gastrointestinal endoscopy are increasing with advancements in the devices used and pediatricians' skills. As part of the Japan Pediatric Endoscopy Study Group, we aimed to investigate the rates of pediatric gastrointestinal endoscopy use and the associated adverse events through a nationwide survey. “…This is due to their easy accessibility, low complication rate and rise in recognition of paediatric gastroenterology as a speciality [ 3 ]. In a study conducted in Japan, the number of institutes showing paediatric gastrointestinal endoscopy use was 4.6 times higher than that stated in the previous reports, and the number of paediatric gastrointestinal endoscopies performed was five times higher [ 4 ]. However, the rate of adverse events was also higher than that previously reported [ 4 ].…”

“…Some diseases of the digestive tract cannot be diagnosed by clinical methods alone, and need to be diagnosed by gastrointestinal endoscopy. At present, digestive endoscopy has been widely used in diagnosis and treatment of children with celiac disease, esophageal stricture, eosinophilic esophagitis, inflammatory bowel disease, congenital muscular hypertrophy and lymphoma, congenital small intestinal lymphangiectasia, polyposis disease, congenital hypertrophic pyloric stenosis, and Hirschsprung's disease (39). The respiratory and circulatory systems are the main sites of AEs in children after GI endoscopy, including fluctuations in heart rate and blood pressure, arrhythmia, myocardial ischemia, respiratory depression, hypoxemia, pulmonary edema, and aspiration (40).…”

“…In this study, although CS did not identify lesions in the colon, three patients were found to have lesions in the terminal ileum, which could be diagnosed via SBCE. Although CS is widely performed on children, is safe ( 9 ), and may lead to a definitive diagnosis by biopsy, the rate of diagnosing a lesion in cases in which the colon is the bleeding source is low. In addition, in severe cases, bowel preparation is often not possible, making the identification of the source of bleeding difficult.…”
