The outbreak of the COVID-19 pandemic produced unprecedented challenges, at a global level, in the provision of cancer care. With the ongoing need in the delivery of life-saving cancer treatment, the surgical management of patients with colorectal cancer required prompt significant transformation. The aim of this retrospective study is to report the outcome of a bespoke regional Cancer Hub model in the delivery of elective and essential colorectal cancer surgery, at the height of the first wave of the COVID-19 pandemic. 168 patients underwent colorectal cancer surgery from April 1st to June 30th of 2020. Approximately 75% of patients operated upon underwent colonic resection, of which 47% were left-sided, 34% right-sided and 12% beyond total mesorectal excision surgeries. Around 79% of all resectional surgeries were performed via laparotomy, and the remainder 21%, robotically or laparoscopically. Thirty-day complication rate, for Clavien–Dindo IIIA and above, was 4.2%, and 30-day mortality rate was 0.6%. Re-admission rate, within 30 days post-discharge, was 1.8%, however, no patient developed COVID-19 specific complications post-operatively and up to 28 days post-discharge. The established Cancer Hub offered elective surgical care for patients with colorectal cancer in a centralised, timely and efficient manner, with acceptable post-operative outcomes and no increased risk of contracting COVID-19 during their inpatient stay. We offer a practical model of care that can be used when elective surgery “hubs” for streamlined delivery of elective care needs to be established in an expeditious fashion, either due to the COVID-19 pandemic or any other future pandemics.

Objective: To confirm the safety and efficacy of outpatient management of laparoscopic appendectomy, with an Enhanced Recovery After Surgery (ERAS) protocol, in adult patients with uncomplicated acute appendicitis. Summary Background Data: Outpatient laparoscopic appendectomy is feasible and secure in selected patients in observational studies. The benefits include reduced length of stay (LOS) and post-operative complications. This is the first Randomized Controlled Trial (RCT) of outpatient management following ERAS protocol. Methods: Patients admitted from the emergency department with acute appendicitis randomized into one of two groups. Standard care within the hospital (HG) or the outpatient group (OG). An ERAS protocol was followed for both groups. Patients in the HG were admitted to the surgical ward. Patients in the OG were referred to the day-surgery unit. The primary endpoint was length of stay. Results: Ninety-seven patients were included, 49 in the OG and 48 in the HG. LOS was significantly shorter in the OG (mean 8.82h) than the HG (mean 43.53h), p<0.001. There was no difference in readmission rates (p = 0.320), we observed only one readmission in the OG. No further emergency consultations or complications were observed. The cost saving was $516.52/patient as a result of the intervention. Conclusion: Outpatient management of appendectomy is safe and feasible procedure in selected patients. This approach could become the standard of care for patients with uncomplicated appendicitis, showing fewer complications, lower LOS and cost. Bevezetés Az appendicitis az egyik leggyakoribb oka az akut hasi műtéti indikációknak. A WHO a SARS-CoV-2- (COVID–19) vírusinfekciót 2020. 03. 11-én világjárvánnyá nyilvánította. A pandémia során hozott, a lakosságot és az egészségügyi ellátókat érintő korlátozó intézkedések során az akut betegellátás is új nehézségekkel nézett szembe. Célkitűzés Saját beteganyagunkból a SARS-CoV-2-világjárvány előtti időszak és a Magyarországon regisztrált COVID–19 III. hulláma alatt az akut appendicitisszel diagnosztizált betegek klinikai tüneteinek, kórlefolyásának, kimenetelének összehasonlítása volt. Módszerek Retrospektív analízisünkbe a COVID–19-pandémia előtti 5 hónap (2019. 10. 16. – 2020. 03. 16.) és a COVID–19 III. hullám (2020. 11. 01. – 2021. 04. 01.) alatt akut appendicitis klinikoradiológiai képével diagnosztizált, 18. életévüket betöltött betegeket vontunk be. Összevetettük a tünetek kialakulása és kórházi megjelenésük közt eltelt idő hosszát, a kialakult appendicitis komplikáltságát, az alkalmazott műtéti módszert, a hospitalizációs időtartamot és a posztoperatív szövődmények kialakulási gyakoriságát. A statisztikai szignifikanciát t-próbával, khi-négyzet-próbával és Fischer-teszttel vizsgáltuk. Eredmények Analízisünkbe 55 beteget vontunk be, 38 főt a pandémia előtti időszakban, 17 főt a COVID–19 III. hulláma alatt. A pandémia ideje alatt az akut appendicitisszel diagnosztizáltak száma csökkenő tendenciát mutatott, viszont a komplikált appendicitis előfordulási aránya növekedett (23,7% → 35%). A tünetek kialakulása és kivizsgálás között eltelt idő a pandémia előtti vizsgált időszakban átlagosan 48,9 óra, míg a COVID–19 III. hulláma alatt 76,3 óra volt (P = 0,285). A hospitalizáció időtartama a pandémia előtt 128,4 és a III. hullámban 142,6 órának adódott (P = 0,327), a posztoperatív szövődmények az alábbiak szerint alakultak: 13,1% → 23,5% (P = 0,435), viszont szignifikáns összefüggés egyik esetben sem volt. Következtetés Analízisünk során a két vizsgált betegpopuláció klinikai paramétereiben, kórlefolyásában, kimenetelében szignifikáns különbséget nem tudtunk igazolni.
