Background:There is a disparity in outcomes between rural and urban emergency medical services (EMS) in both developed and developing countries. However, there is a scarcity of research that directly asks EMS staff in both rural and urban areas of developing countries how service delivery could be improved. The aim of the present study is to gain insights from frontline workers regarding organisational factors that may underpin discrepancies between rural and urban EMS performance.Subject and Methods:The study was undertaken in the Riyadh region of Saudi Arabia, with 20 participants (10 rural, 10 urban) drawn from Saudi Red Crescent EMS. Semi-structured interviews were undertaken between October 2019 and July 2020. All interviews were conducted in Arabic, transcribed verbatim and translated into English. Results:Data analyses identified key organisational factors that resulted in barriers and impediments for EMS staff. Differences and similarities were observed between rural and urban respondents, with identified issues including response and transportation time, service coordination, reason for call-out, as well as human and physical resourcing.Conclusion:The findings identified key issues impacting on EMS performance across both rural and urban areas. In order to address these problems, changes are recommended. These recommendations include a comprehensive review of rural EMS vehicles, with a particular focus on the age; incentives to improve the numbers of paramedics in rural areas and more localised specialist training opportunities for rurally-based personnel; and the implementation of national public education program focusing on the role of the EMS.

Purpose: The pandemic makes everyone alert, including nurses who are in emergency rooms at community health centers, the importance of their experiences is a lesson for nurses to maintain themselves and be effectivein providing services. This study aimed to explore the experience of nurses who are in emergency rooms at rural area during the coronavirus disease pandemic. Methods: This qualitative research was conducted based on the Nvivo 12 analysis method using in-depth semistructured interviews. Data saturation was reached after 20 interviews were completed. Data collection lasted for 1 month from February to March 2020. Results: Semistructured interviews with 20 nurse participants obtained the following participant characteristics. Eight participants were males and 12 were females with an age ranged from 28 to 43 years (average age 36.4 years). The majority had vocational education (75%), with long experience that ranged from 5 to 15 years (average 11 years). The findings of four themes and seven subthemes. The theme of the findings is Expressions of care, Compliance increases using Personal Protective Equipment (PPE), Focus to refer to, Triage at the forefront. Conclusion: This research reveals that Expressions of care, Compliance increases using PPE, Focus to refer to, Triage at the forefront is the main theme identified in this study. Further investigation of the readiness of nurses in handling patients in the emergency room is considered to be of benefit to the results of this study.

PurposeSaudi female paramedics face many challenges in the current Saudi emergency medical service (EMS). This study aimed to investigate perceptions of EMS experts, leaders, managers and academics about the challenges faced by Saudi female paramedics in the Saudi Arabian EMS workforce.Design/methodology/approachA descriptive qualitative research approach was used employing semi-structured face-to-face interviews with seven EMS leaders, managers and academics in Riyadh, Saudi Arabia. Data were analysed using thematic analysis informed by the work of Braun and Clark.FindingsThree themes emerged from the interviews that described EMS experts' perceptions, namely, cultural and family challenges, the value of Saudi female paramedics in the workforce and workforce issues.Originality/valueOverall, the interviews revealed that Saudi females faced several challenges that could affect their job duties and capacity to work in the EMS. Currently, Saudi females face difficulties with family, social and religious responsibilities, such as taking care of children and homes. Workforce issues were also considered problems that affect Saudi female paramedics in the workplace, such as physical fitness and psychological burnout. The EMS leaders, managers and academics emphasised that due to the new Saudi 2030 vision, it is expected and recognised that more females need to be employed in the EMS workforce.
