National Estimates of the Quantity and Cost of Informal Caregiving for the Elderly with Dementia*

Abstract: OBJECTIVE:Caring for the elderly with dementia imposes a substantial burden on family members and likely accounts for more than half of the total cost of dementia for those living in the community. However, most past estimates of this cost were derived from small, nonrepresentative samples. We sought to obtain nationally representative estimates of the time and associated cost of informal caregiving for the elderly with mild, moderate, and severe dementia. “…Composite scores for ADL difficulty were computed and cut-offs determined a priori to represent no limitations (0), mild/moderate limitations (1-2), and severe limitations (3 or more). The HRS tests cognitive function using well-described methods; [29][30][31][32][33] final models used a binary measure to describe patients as having no cognitive impairment versus any cognitive impairment. Two binary measures related to disability were obtained from the HRS data: whether the patient required nursing home services during the most recent HRS interview, or had used home health care services in the 2 years prior to the interview.…”

“…Time costs were derived from a combination of the total number of hours spent caregiving and a value for each 20 Time spent by caregivers supporting cancer survivors represents time not spent pursuing usual activities, including work and leisure. In economics, the value of these activities is referred to as the opportunity cost of providing informal care.…”

“…The HRS uses a performance-based measure of cognition, which has been validated at the national level (25) and which enables classification of different levels of impairment, (based on the Aging, Demographics, and Memory (26,27). Although most practitioners are familiar with and can recognize dementia, it is not clear that mild cognitive impairment is as readily identified.…”
