Background. Nasopharyngeal cancer is rare malignant tumor of childhood. The clinical signs of this disease are rhinolalia, nasal breathing difficulties, headache and lymphadenopathy. There are no clinical cases of this nosology described previously in Russian literature. Clinical Cases Description. 1. Patient T., 11 years old, was moved from otolaryngology to oncology department due to CT findings of lump in the nasopharynx. At admission the child had complaints on fatigue, nasal stuffiness, headache for past 3 months, pain in the neck at turning of his head. The histological study of this lump closing choanae at both sides was performed -nasopharyngeal carcinoma was revealed. 2. Patient A., 14 years old, was hospitalized in oncology department after self-admission to pediatric oncologist with complaints on rhinolalia for a month and jugular glands increase on both sides, weight loss (2 kilos) during last 3 months. Prior to this he was managed on outpatient basis and in infectious diseases department with the diagnosis "Infectious mononucleosis. EBV associated (antibodies (IgG) to capsid antigen of Epstein-Barr virus -5.4 copies/ml), atypical moderate course". The generalized lymphadenopathy was revealed during examination in the oncology department. Multiple conglomerates of lymph nodes groups were revealed on CT scans. The histological study of biopsy sample from affected lymph node has shown metastases of nasopharyngeal carcinoma. The therapy according to the NPC-GPOH-2003 protocol was prescribed with marked positive dynamics after 3 months. Conclusion. The presence of headache, nasal breathing difficulties, body weight loss, fever in combination with lymphadenopathy in the neck requires caution regarding nasopharyngeal cancer and in differential diagnosis with any other infectious or inflammatory diseases.

Nasopharyngeal carcinoma (NPC) is a rare form of malignancy, accounting for 2% of all cancers of the head and neck in Europe. Axillary lymph node metastases are very rare in these cases. This is a case report of a 40-year-old premenopausal woman diagnosed in May 2015 with T1N2M0 stage III NPC, treated with induction chemotherapy, followed by chemo-radiotherapy. Post-therapeutic computed tomography (CT) scan showed partial response (PR) on the primary tumor and complete response (CR) on the latero-cervical lymph nodes. In 2017, our patient developed left carcinomatous-like mastitis with axillary lymphadenopathy. This raised suspicions of a carcinomatous mastitis. The pathology report with immunohistochemistry (IHC) of the third biopsy highlighted axillary metastasis of a non-keratinizing squamous cell carcinoma (NSCC). There are very few references in the literature regarding axillary metastases from squamous cell carcinoma of the head and neck (HNSCC). As far as we know, this is the first case report of mastitis due to NPC. To conclude, treatment consisted of two surgical excisions of axillary lymphadenopathy associated with local radiotherapy and chemotherapy (neo-adjuvant, adjuvant). The second surgery, performed after radiotherapy, required plastic surgery. A psychiatric evaluation was necessary, revealing a reactive anxiety disorder. This case required multidisciplinary management, where oncology, plastic surgery, pathology and psychiatric specialists collaborated in deciding the therapeutic approach.

Purpose: No standard radiotherapy regimens was established in the treatment of de novo metastatic nasopharyngeal carcinoma (mNPC) with bone-only metastasis. The current study aimed to investigate the efficacy of palliative chemotherapy (PCT) plus locoregional radiotherapy (LLRT) with or without local radiotherapy (RT) to bone metastatic lesions in mNPC, and identify the optimal candidates.Methods: We retrospectively analyzed 141 de novo mNPC patients with bone-only metastasis who received at least two cycles of PCT with or without LLRT and RT to bone metastasis. The difference in survival was evaluated by the log-rank test. Univariable and multivariable analysis was made by Cox regression. Results: Patients who received PCT plus LLRT had significantly longer overall survival (OS) (45.0 months vs 13.5 months, HR = 0.30 , p = 0.001) and progression-free survival (PFS) (29.0 months vs 11.0 months, HR = 0.34, p = 0.014), especially in patients who had less than 3 metastatic bone lesions. Multivariate analysis confirmed that LRRT, more chemotherapy cycles (≥ 4) and limited number of bone metastasis (≤ 3) were favorable prognostic factors for OS. Subgroup analysis revealed that RT to metastatic bones had a tendency to prolong the survival time in the unselected population who received PCT plus LLRT (p > 0.05), while further data suggested that RT to metastatic bones dramatically improve OS (72.0 months vs 26.0 months, p = 0.002) and PFS (60.0 months vs 20.0 months, p = 0.006) for mNPC with less than 3 metastatic bone lesions.Conclusions: LLRT and RT to bone metastatic lesions followed by PCT in de novo mNPC with bone-only metastasis significantly prolonged survival in patients with less than 3 metastatic bone lesions.
