Nasogastric Tube Feeding in Older Patients: A Review of Current Practice and Challenges Faced

Abstract: Nasogastric tube feeding is an essential way of delivering enteral nutrition when the oral route is insufficient or unsafe. Malnutrition is recognised as a reversible factor for sarcopenia and frailty. It is therefore crucial that malnutrition is treated in older inpatients who have dysphagia and require enteral nutrition. Despite five National Patient Safety Alerts since 2005, “Never Events” related to nasogastric feeding persist. In addition to placement errors, current practice often leads to delays in feed… Show more

“… 34 It was a preferable option to select the nasogastric feeding tube when facing uncorrected disease, such as decline in cognition after stroke, dysphagia and so on. 35 Moreover, it is safe and effective to deliver nutrients and/or fluids to the gastrointestinal tract through the nasogastric feeding tube. 36 This idea is supported by many studies, which have shown nutritional support treatment of nasogastric feeding tube played a central role in the management of elderly who were malnourished or at high risk of malnutrition, and significantly improved nutritional status.…”

“…The fine-bore, flexible nasogastric feeding tube has been used in elderly patients > 65 years of age with malnutrition or dysphagia [ 42 ] as a short-term solution for enteral feeding. This method has been seen to provide benefit to prevent malnutrition but has risk of dislodgement and potential delay in utilizing the NGT while awaiting placement confirmation by x-ray [ 43 , 44 ].…”

“…A dysfunctional oral cavity may lead to poor oral hygiene care (10) and negatively impact the oral microbial community, further complicating patients’ health condition. Although NG tube-feeding could be beneficial for the patients in the short term to deal with malnutrition (11), patients with prolonged NG-tube indwelling often suffered from dysphagia (12), malnutrition (13), and aspiration pneumonia (14).…”
