Neurological diseases continue to increase in prevalence worldwide. Combined with the lack of modifiable risk factors or strongly efficacious therapies, these disorders pose a significant and growing burden on healthcare systems and societies. The development of neuroprotective or curative therapies is limited by a variety of factors, but none more than the highly selective blood-brain barrier. Intranasal administration can bypass this barrier completely and allow direct access to brain tissues, enabling a large number of potential new therapies ranging from bioactive peptides to stem cells. Current research indicates that merely administering simple solutions is inefficient and may limit therapeutic success. While many therapies can be delivered to some degree without carrier molecules or significant modification, a growing body of research has indicated several methods of improving the safety and efficacy of this administration route, such as nasal permeability enhancers, gelling agents, or nanocarrier formulations. This review shall discuss promising delivery systems and their role in expanding the clinical efficacy of this novel administration route. Optimization of intranasal administration will be crucial as novel therapies continue to be studied in clinical trials and approved to meet the growing demand for the treatment of patients with neurological diseases.

Introduction Nasal septal deviation (NSD) and concha bullosa (CB) are associated with airway obstruction in mouth breathers. Mouth breathing is associated with alterations in maxillary growth and palatal architecture. The aim of our study was to determine the effect of the presence of CB and NSD on the dimensions of the hard palate using cone-beam computed tomography (CBCT). Materials and methods A retrospective study was conducted using CBCT scans of 200 study subjects. The study subjects were divided into four groups based on the presence of CB and NSD. Septal deviation angle (SDA), palatal interalveolar length (PIL), palatal depth (PD) and maxillopalatal arch angle (MPAA) were measured in the study groups. Results The presence of NSD and CB was associated with significant (p < 0.001) differences in the palatal dimensions of the study subjects. The PIL and MPA (p < 0.001) were significantly reduced (p < 0.001), whereas the PD was significantly increased (p < 0.001) in study subjects with NSD and CB. There was no significant change in the palatal dimensions between the unilateral and bilateral types of CB. Among the palatal dimensions, the PIL had the most significant association (R2 = 0.53) with SDA and CB. There was a significant correlation between the palatal dimensions and SDA when CB was present along with NSD. Conclusion Based on the results of this study, it can be concluded that the presence of NSD and CB have a significant effect on the palatal dimensions and, therefore, they may be associated with skeletal malocclusion.

It is known that deviation of the septum plays an etiological role in the obstructions formed in the osteomeatal complex. In this study, it was aimed to evaluate the effect of the degree of deviation of the nasal septum on the distances between the bone structures forming the osteomeatal complex. Method: The research universe was obtained by retrospectively scanning 1200 computed tomography (CT) images of January 2018-December 2019 in the PACS archive of the Department of Radiology of Sivas Cumhuriyet Faculty of Medicine Hospital. A total of 453 individuals, 228 females and 225 males (18-82 years old) meeting the criteria for inclusion in the study were included. Septal deviation angle was measured. Individuals were grouped according to the degree of septal deviation. Osteomeatal complex spaces (OMC) were evaluated according to the degree of septal deviation. In the study, the distance between OMC spaces was also investigated in the presence of sinusitis. Results: In the septum, the distance between lamina papyricea and middle nasal concha (LP-MNC) at medium and high grades was found to be large in the opposite direction of the deviation. The distance between the lamina papyricea and uncinate process [ethmoidal infundibulum (LP-UP)] was small on the right side in low-grade deviation to the right. When the distance between uncinate process and middle nasal concha (UP-MNC) and the distance between LP-UP was compared, it was found to be smaller in the direction of deviation in individuals with middle deviation to the left (p < 0.05). While the right osteometal complex spaces were found to be smaller than the control group, the distance between UP-MNC from the left osteomeatal spaces made a difference with the control group. Conclusions: Degrees and type of septal deviation were effective in narrowing the osteomeatal complex spaces. It was determined that OMC distances did not change with the presence of sinusitis. It is thought that it may be useful to evaluate the degree of septal deviation in obstruction of osteomeatal complex spaces and mucosal changes in the paranasal region.
