Nailfold capillaroscopic findings in primary Sjögren’s syndrome with and without Raynaud’s phenomenon and/or positive anti-SSA/Ro and anti-SSB/La antibodies

Abstract: The aim of this study was to assess nailfold capillaroscopic (NC) findings in patients with primary Sjögren's syndrome (PSS) with and without Raynaud's phenomenon (RP) as well as in the presence of positive anti-SSA/Ro and anti-SSB/La antibodies. Videocapillaroscopy was performed in 150 patients with PSS. Data collected included demographics, presence of RP, PSS symptoms, antinuclear antibodies, rheumatoid factor, anti-Ro, anti-La, anti-CCP, salivary scintigraphy, labial biopsy, and NC findings. RP was present… Show more

“…The percentages of patients with pSS, SLE, and MCTD in whom an SSc pattern was found on NCM were in line with previous findings 9,10,11,12,14,32 . Only for the patients with RA was the percentage of patients with an SSc pattern on NCM lower than expected 16 .…”

“…A recent review shows an association between abnormal NCM findings in SLE and disease activity 13 . About 10% of patients with pSS have SSc-like abnormalities on NCM 14,15 . Twenty-one percent of the patients with RA had SSc-like NCM abnormalities, although it is unknown how many of these patients experienced RP symptoms 16 .…”

“…It is commonly used for differentiating primary from secondary Raynaud's phenomenon and for the diagnosis of sclerodermaspectrum disorders, including systemic sclerosis (SSc), dermatomyositis (DM), undifferentiated CTD, and mixed CTD (1). NFC changes can also be detected in other chronic autoimmune rheumatic diseases, namely systemic lupus erythematosus (SLE), Sjogren's syndrome, rheumatoid arthritis, and antiphospholipid syndrome (2)(3)(4)(5).…”

“…Considering these data, the observation of positive NVC only in patients with anti Ro/SSA 52k positivity without associated RP could not be random. NVC was reported positive only in about 10% of patients with SjS [17], but increasingly data suggest NVC as a useful diagnostic tool for myositis, even in patients with SSA and without RP [18]. Therefore, a silent myositis also in patient with SjS could be hypothesized.…”
