A 60-year-old white man presented to the emergency department with acute colicky right fl ank pain. His medical history included chronic renal insuffi ciency, hypertension, hyperlipidemia, and chronic lower back pain. The patient also noted that he had been experiencing sacral pain over the past several months that worsened when sitting and was relieved when standing. In addition, he experienced occasional bouts of constipation during this time. He reported no history of bowel or bladder incontinence, motor weakness, or sensory paresthesias.
