Myoclonus From Intoxication by Bismuth Iodoform Paraffin Paste (BIPP) Nasopharyngeal Packing

Abstract: Bismuth iodoform paraffin paste (BIPP) gauze is widely used as an antiseptic wound packing in otolaryngology, head, and neck surgery. Uncommonly, BIPP can cause intoxication. Our report highlights an elderly patient who developed encephalopathy and overt myoclonus after nasopharyngectomy secondary to intoxication by the components of the BIPP gauze. The patient's impaired renal function, the amount of BIPP packing and the extensive nature of his wound likely predisposed him to BIPP toxicity. The myoclonus and … Show more

“…O'Connor AF reported iodoform toxicity when BIPP was used in packing large defects caused by maxillectomy [ 17 ]. Wilson stated that BIPP, which is used to fill cavities in the mandible, can cause personality changes and insomnia due to encephalopathy after more than seven weeks of use [ 18 ]. The sensitivity to bismuth sulphate or iodoform in bismuth iodoform paraffin paste (BIPP) is type IV delayed hypersensitivity reaction, largely involving T cells and macrophages.…”

“…In this case, the toxicity was associated with several factors, such as the high extension of the wound, the amount of BIPP packing, and the impaired renal function. After the removal of the BIPP packs, the patient gradually improved over the next two weeks without further intervention (Table 2, Entry 8) [121].…”

“…However, neuropsychiatric symptoms can develop from the use of bismuth iodoform paraffin paste gauze. In order to prevent it, clinicians should take into account the patient’s renal and hepatic function as well as the size and condition of the wound [ 362 ].…”
