Myocardial Inflammation, Sports Practice, and Sudden Cardiac Death: 2021 Update

Abstract: Myocardial inflammation is an important cause of cardiovascular morbidity and sudden cardiac death in athletes. The relationship between sports practice and myocardial inflammation is complex, and recent data from studies concerning cardiac magnetic resonance imaging and endomyocardial biopsy have substantially added to our understanding of the challenges encountered in the comprehensive care of athletes with myocarditis or inflammatory cardiomyopathy (ICM). In this review, we provide an overview of the curren… Show more

“…Even for inflammation, supporting evidence is limited to isolated case reports [ 43 ]. However, inflammation may render the ventricular myocardium more vulnerable to VAs [ 44 ], and this pathophysiological pathway may be especially relevant in elite athletes, who are at risk of viral infections due to frequent international travels, outdoor training competitions, exposure to extreme climates, and exercising despite symptoms of common cold, which may all facilitate the development of myocarditis [ 45 , 46 ], especially in the COVID-19 era [ 47 ].…”

“…An higher incidence of sport related SCD in case of myocarditis is documented[ 148 ], for the higher rate of life-threatening ventricular arrhythmias[ 150 ]. In acute form, echocardiography may show global LV dysfunction[ 151 ], wall motion abnormalities and pericardial effusions[ 15 , 152 , 153 ], an diffuse myocardial edema (increased wall thickness)[ 154 - 157 ]. In healed form, echocardiographic LV systolic dysfunction is found rarely because there is often small myocardial fibrosis, with undetectable scar at echocardiography[ 158 ]: Its evaluation still represent one of the bigger challenges of sport cardiology.…”

“…The next challenge is to translate all the results in the field of exercise to the clinics. In particular it will be necessary to better clarify the definition of "intense/strenuous exercise" in patients, to be able to provide clear, and maybe patient-tailored, recommendation to balance between a good quality of life and the risk of sudden cardiac death (137,138).…”
