Myeloid sarcoma is defined as a tumor mass of immature myeloid cells that may be observed in a variety of locations including bone, skin, lymph nodes and soft tissues. However, oral involvement of myeloid sarcoma is extremely rare. These tumors are considered as specific lesions of acute myeloid leukemia. We present a case of a myeloid sarcoma of the upper vestibular gingiva in a 29-year-old woman who has no hematologic disease history. Multiple metastases were found in floor of the nasal cavity, left breast, and left lacrimal gland 12 months after primary diagnosis.

A case of neutrophilic granulocytic sarcoma and a case of acute basophilic leukemia are described. The former was diagnosed in an 8-year-old Holstein cow with a large intrathoracic tumor mass (case 1), and the latter in a 14-month-old Holstein heifer with a rapid increase in white blood cell count (case 2). In case 1, the tumor mass was composed of agranular cells and more mature cells with eosinophilic granules staining positive for naphthol AS-D chloroacetate esterase (CAE) and macrophage, myeloid/ histiocyte antigen (clone MAC387). In case 2, the tumor tissues consisted chiefly of myeloblastoid cells. However, differentiation to myelocytoid or metamyelocytoid cells was observed, and their intracytoplasmic eosinophilic granules were positive for tryptase (a marker for basophils and mast cells), but not for CAE (a marker for neutrophils and mast cells). Although reports of myeloid neoplasms are rare in cattle, as documented here, it is not difficult to diagnose them on the basis of histology, histochemistry and immunohistochemistry.

Introduction: Myeloid sarcoma is an unusual extramedullary tumorous lesion consisting of immature cells of granulocytic series. Peritonsillar swelling as a manifestation of myeloid sarcoma is quite unique. The clinical diagnosis of this entity is difficult and fine needle aspiration cytology might be a useful alternative in centres with limited facility. We present such a case which clinically can mislead us as the presentation is similar to that of a peritonsillar abscess.Case report: A 30 year old male presented with swelling inside the mouth with difficulty in swallowing and fever for last 4 days. The patient was clinically evaluated and diagnosed as suffering from peritonsillar abscess. Failed pus aspiration from the intra-oral swelling as well as firmness of the mass directed to go for a fine needle aspiration cytology examination as well as radiological evaluation. Correlating with the radiologic finding of effaced tissue architecture and the presence of blast from the aspirate as well as peripheral blood picture, a diagnosis of myeloid sarcoma was made. Subsequently the patient was sent to the haematologist Conclusion: As the symptoms are clinically similar to locally occurring lesions, high level of suspicion is needed to diagnose such a case especially when conventional therapy fails to treat such conditions.
