Mycotic Aneurysm of the Internal Carotid Artery

Abstract: A 17-year-old man presented with signs of rupture of a high cervical internal carotid aneurysm. Following angiographic demonstration of the aneurysm, he was immediately treated by balloon catheter occlusion. Eight days later the ligation of the supraclinoid portion of the internal carotid on the left was prompted by the formation of blood clots and emboli from this segment. The patient tolerated both procedures well.

“…Other than our case, 5 case reports of carotid mycotic aneurysm as a complication of underlying dental infection or extractions were found. [43][44][45][46][47] The mortality rate was 17% for these cases with a mean follow-up of 5 1 /2 months. The first report was that of an 11-year-old boy with an extensive Penicillium infection and associated rupture of his aneurysm, which developed after a maxillary tooth extraction.…”

“…Reported treatments of microbial pseudoaneurysms are balloon catheter occlusion, 8 ligation, resection, and reconstruction. [10][11][12] As several complications were reported after carotid ligation, ' l an anastomosis between the intracranial internal carotid artery and the extracranial internal carotid artery was added to the ligation in our patient.…”
