Mycoplasma pneumoniae detection with PCR in renal tissue of a patient with acute glomerulonephritis

Abstract: Renal disease concurrent with a Mycoplasma pneumoniae infection is uncommon. In this report we describe the clinical outcome of a 6-year-old patient who presented with a rapidly progressive glomerulonephritis that required dialysis. A kidney biopsy was performed, and the results revealed membranoprolipherative glomerulonephritis. The IgM serology was positive, and M. pneumoniae DNA was detected in a renal biopsy sample using a nested-PCR assay. The outcome was good.

“…Acute glomerulonephritis of various types, nephrotic syndrome, interstitial nephritis, and IgA nephropathy have occasionally been described in association with M. pneumoniae infection, presumably as a result of immune complex formation. However, the organism has also been detected by PCR from a kidney biopsy specimen of a child with membranoproliferative glomerulonephritis who also had elevated IgM (263) and by immunoperoxidase staining of kidney tissue of a patient with acute interstitial cystitis (264). Renal complications can be of great clinical importance since they can lead to renal failure (265,266).…”

“…Acute glomerulonephritis, including nephrotic syndrome, interstitial nephritis and immunoglobulin (Ig)A nephropathy, has been associated with MP infections and is presumed to be secondary to immune complex formation [ 5 ]. There are reports documenting detection of the MP organism itself by PCR and immunoperoxidase staining from kidney tissue [ 60 , 61 ].…”

“…An abdominal US scan revealed enlarged kidneys with enhanced echogenicity. No complications or recurrences were observed after the treatment ended (30) . Acute interstitial nephritis in a child with an MP infection was presented by Hinokiyama et al A 15-year-old patient with cough and fever developed haematuria and proteinuria, and a renal biopsy confirmed ATIN.…”
