Multisite Parent-Centered Risk Assessment to Reduce Pediatric Oral Chemotherapy Errors

Abstract: Parents of pediatric oncology patients readily participated in a proactive risk assessment method, identifying processes that pose a risk for medication errors involving home oral chemotherapy.
