Epileptik nöbetler multipl skleroz (MS) hastalığının seyrinde görülebilmektedir. Ancak MS'nin ilk semptomunun nöbet olması son derece nadirdir. Bu yazıda ilk atağı epilepsi nöbetiyle ortaya çıkan 26 yaşında MS'li bir kadın sunulmaktadır. Hasta acile jeneralize tonik klonik nö-betle başvurdu. Nörolojik muayenede sağ hemiparezi bulguları ve hiperaktif derin tendon refleksler dışında özellik saptanmadı. Kraniyal manyetik rezonans görüntülemesinde periventriküler yerleşimli, çok sayıda milimetrik lezyon ve sol sentrum semiovalede 45x27 mm çaplı, yarım halka şeklinde kontrast tutan, kortikal, T2/FLAİR kesitlerde hiperintens lezyon izlendi. Beyin omurilik sıvısı incelemelerinde IgG indeksi yüksek, oligoklonal band (+) idi. Elektroensefalografide sol hemisfer fronto-temporal bölgede belirgin keskin dalga paroksizmleri gözlendi. Multipl skleroz tanısı konuldu ve pulse kortikosteroid verildi. Tekrarlayan nöbetleri nedeniyle hastaya antiepileptik tedavi başlandı. Nöbetler monoterapi ile kontrol altına alındı. Multipl sklerozlu hastaların nöbet geçirebildiği bilinmektedir. Nöbetle başvuran özellikle genç olgularda ayırıcı tanıda multipl skleroz mutlaka akla getirilmelidir.Anahtar sözcükler: Epilepsi; multipl skleroz; nöbet. SummaryWhile epileptic seizures are seen in the course of multiple sclerosis, they are rarely the first symptom. The first epileptic seizure of a 26-yearold woman with multiple sclerosis is described in the present report. The patient presented to the emergency department with generalized tonic-clonic seizure. Neurologic examination was normal except for right-sided hemiparesis and hyperactive deep tendon reflexes. Cranial MRI revealed periventricular, multiple millimetric lesions and a 45x27-mm, semi-ring-enhanced, cortical, T2/FLAIR, hyperintense lesion in the centrum semiovale. IgG index was high, and oligoclonal band was positive in cerebrospinal fluid examination. Electroencephalography showed prominent fronto-temporal activity on the left side and sharp wave paroxysms. Multiple sclerosis was diagnosed, and pulse corticosteroid therapy was initiated. Due to recurrent seizures, antiepileptic drug was added to treatment; seizures were controlled with monotherapy. It is known that patients with multiple sclerosis experience seizures. Multiple sclerosis should be considered in the differential diagnosis of young patients presenting with seizures.

There is accumulating evidence of a mutual relationship between multiple sclerosis and structural epilepsy. Comorbidity of these severe neurological disorders is reported in numerous recent publications in the available foreign literature. Modern diagnostic tools include comprehensive neurological examinations as well as computed tomography, electroencephalography and 18F FDG positron emission tomography. In this paper we report the case of a female patient with multiple sclerosis and structural epilepsy who was admitted to the clinic due to weakness of left limbs, difficulty in walking, and blurred vision in the left eye. The neurological examination showed small horizontal nystagmus, left-sided hemianopia, quadrihyperreflexia, nonsustained clonus of both feet, left-sided latent hemiparesis, paresthesia in the left limbs. Electroencephalography (EEG) revealed paroxysmal activity on the left side, MRI indicated evidence of cerebrospinal MS with activity .18F FDG PET scans were normal. Based on the literature data and our own case report we could draw the conclusion that as epileptic seizures in MS patients may occur due to inflammatory plaques of demyelination it could be suggested that there exists a correlation between MS and structural epilepsy.

Abstract: Our study confirms an increased risk of epileptic seizures in MS patients. It underlines that seizures may be the first observable symptom in MS and the frequency and seriousness of status epilepticus.
