“…A cortical strut graft will very likely not be able to address the extensive forces applied on the proximal femur without this form of stability. Based on the results with internal fixation in other studies [1, 4, 5, 8, 10, 1214, 17, 20, 2325] and the findings from our present study, we recommend primary internal fixation in patients with extensive lesions that threaten to fracture or have already induced a fracture, because this approach has shown promising results in several studies. More importantly, although only the combination of internal fixation and autografts has so far been studied, there might be an important role for implantations in combination with cortical allografts in patients with preoperative risk factors.…”

“…[16][17][18] Guille Kumar and MacEwen 19 reported that none of the lesions in their series was eradicated or diminished in size, and that all bone grafts were absorbed. Curettage and bone grafting is not suitable in patients with deformity and pathological fracture.…”

“…osteotomies and Zickel nail fixation. 20 Freeman et al 16 recommended this technique because it provided intramedullary stabilisation, reduced the stress concentration distally and, when combined with osteotomy, can correct deformity. However, Guille et al 19 reported a patient with fibrous dysplasia treated with a Zickel nail who at longterm follow-up demonstrated thinning of the medial cortex, probably caused by stress shielding resulting from the intramedullary nail.…”

“…Connolly 62 and Freeman et al 63 reported on five patients with extensive polyostotic fibrous dysplasia in whom a proximal femoral deformity extending well distal to the femoral neck had been treated with multiple osteotomies and intramedullary Zickel nail fixation. The Zickel nail allowed stabilization of the entire femur and femoral neck, with fewer failures of fixation or progressive deformities.…”
