Multiphasic and multifocal cryptococcal immune reconstitution inflammatory syndrome in an HIV-infected patient: interplay of infection and immunity

Abstract: We report a case of cryptococcal immune reconstitution inflammatory syndrome affecting the lungs, and 10 months later the cervical lymph nodes, in the absence of cryptococcal meningitis, in advanced HIV infection. Our report demonstrates the organ-specificity of the timing of the inflammatory response and illustrates the organ-specific interplay of immunity and infection in cryptococcal disease.

“…Cryptococcal IRIS may manifest as relapsing aseptic meningitis, increased intracranial pressure, new focal neurologic signs, intracranial cryptococcomas, lymphadenopathy, and/or development of abscesses [11, 19, 5254] which can present as multifocal or multiphasic disease [55*]. Mortality associated with cryptococcal IRIS ranges from 0 to 67% [19, 20, 50].…”

“…1 Antifungal therapy was enhanced with liposomal amphotericin B and 5-flucytosine in a patient with localized paradoxical pulmonary C-IRIS (Table 1). 7 We did not intensify the antifungal therapy in our patient as evidence of active cryptococcal infection was not found when pulmonary C-IRIS was diagnosed. Corticosteroids were shown to be very effective in this case.…”

“…This search strategy yielded a total of 36 studies, among which five were relevant studies. [7][8][9][10][11] Reviewing of bibliographies and citations of those five literatures yielded another additional three relevant literatures. [12][13][14] Seven candidate cases of paradoxical pulmonary C-IRIS were identified from those eight relevant literatures.…”

“…On the other hand, paradoxical cryptococcal IRIS does not require a new course of induction and consolidation therapy, because the pathogens are no longer viable. Many clinicians end up giving standard antifungal therapy while waiting for negative cultures [ 6 ], which also occurred in our case. Adjunctive therapy with steroids can be considered in patients with severe paradoxical responses.…”
