ZusammenfassungDie abdominelle Sepsis ist durch ein lokales intraabdominelles Geschehen (Peritonitis) und die extraperitoneale Allgemeinreaktion (Sepsis) gekennzeichnet. Damit sind die Therapiekonzepte klar definiert. Die Kausaltherapie besteht in der Ausschaltung der primären Infektionsquelle (chirurgische Herdsanierung). Nur wenn sie gelingt und eine permanente Reinfektion des Bauchraums verhindert wird, können supportive Maûnahmen der Intensivmedizin und aktuelle adjunktive Therapieansätze überhaupt zum Tragen kommen. Eine modifizierte operative Therapie in Richtung eines weniger aggressiven Vorgehens (On Demand-Relaparotomie), die Entwicklung hochwirksamer Antibiotika, der konsequente Einsatz moderner Beatmungsverfahren und die rasche initiale Kreislaufstabilisierung (Volumentherapie), wenn möglich unter Verzicht auf Katecholamine, haben in gleichem Maûe dazu beigetragen, dass heute die Mortalität der abdominellen Sepsis in Zentren unter 15 % liegt. Eine weitere Prognoseverbesserung ist denkbar durch spezifische Ansätze einer adjunktiven Sepsistherapie, die z. Zt. aber noch nicht in gewünschtem Umfang zur Verfügung steht.

Succeeding in this attempt and preventing a permanent reinfection of the abdomen are the requirements to successfully start intensive care measures and current adjunctive therapies. A modified operative therapy aiming at less aggressive modalities (on demand relaparotomy), the development of highly effective antibiotics, the consequent use of modern ways of ventilation and the fast stabilisation of the circulation (volume replacement) without using catecholamines have all contributed to a mortality rate in patients with abdominal sepsis of less than 15 % in centers of excellence. Further improvement of the prognosis could be achieved by specific concepts of therapeutical strategies in the treatment of sepsis. But currently these therapies are not largely available.
