Multimodal Analgesia, Current Concepts, and Acute Pain Considerations

Abstract: Alpha 2 agonists have been shown to have opioid-sparing effects, but can cause hypotension and bradycardia and must be taken into consideration when administered. Acetaminophen is commonly used in a multimodal approach, with recent evidence lacking for the use of IV over oral formulations in patients able to take medications by mouth. Studies involving gabapentinoids have been mixed with some showing benefit; however, future large randomized controlled trials are needed. Ketamine is known to have powerful anal… Show more

“…For example, the overall use of opioids and the opioid-sparing effect of meloxicam IV were greater in orthopedic surgeries than in other surgery types. The use of multimodal and complementary medications to reduce opioid consumption is a goal cited by recent guidelines, 11,33 and the magnitude of the opioid-reducing effect observed in this study is clinically meaningful within this context.…”

“…In order to deal with opioid crisis happening during traditional pain management, concept of multimodal analgesia is introduced. Local anesthetics (LAs) is one of the most frequently-used and safest analgesics in multimodal regimens [2,3]. However, limited duration (less than 24 h) and potential toxicity (cardiac and central nervous malfunction) restrict its application and raise the urgency to counterbalance the side effects and prolonged analgesia [4][5][6].…”
