Multimodal Analgesia Bundle and Postoperative Opioid Use Among Patients Undergoing Colorectal Surgery

Claes Gedda,
Jonas Nygren,
Anna Garpenbeck
et al.

Abstract: ImportanceA key objective in contemporary surgery is to reduce or eliminate the usage of opioids to minimize gastrointestinal adverse effects, fatigue, and long-term opioid dependency.ObjectivesTo evaluate the association of the implementation of a care bundle of 3 opioid-sparing interventions with the amount of opioids consumed postoperatively among patients undergoing major abdominal surgery and to examine the respective associations of the 3 components.Design, Setting, and ParticipantsThis retrospective coh… Show more

“…While opioid administration may vary depending on the type of anastomosis performed, the postoperative pain management strategies within the ERAS protocols are adaptable and diverse. Therefore, even with advancements in surgical techniques, the potential improvement in laparoscopic surgical outcomes by opioid-sparing approaches remains noteworthy 40 . Additionally, less pain allows the patient to mobilize and walk around the ward early, which may promote bowel movements.…”

“…Nonetheless, due to its broader action spectrum, clonidine may heighten hypotension risk compared to dexmedetomidine [ 95 ]. Intriguingly, evidence suggests a correlation between clonidine usage and increased opioid consumption in colorectal surgery patients—a point of concern considering the adverse outcomes associated with elevated opioid use [ 96 ]. These findings underscore the nuanced balance required in managing postoperative pain.…”

“…The default change is a relatively blunt instrument to impact prescribing habits, but it is effective in pushing broad swaths of prescribers towards lowering quantities of opioid prescriptions. The use of multimodal analgesia at different points in the perioperative period has been shown in multiple clinical scenarios to reduce the use of postoperative opioids [20][21][22][23] and incorporating multimodal analgesia plans into surgical patient care can pair with a lower default count to reduce the quantities of postoperative opioids needed. In the future, default changes could be combined with prescribing recommendations and multimodal analgesia plans targeted to specific patients and procedures in order to find the optimal pain control for each individual patient and operation.…”
