Multifocality in Sporadic Medullary Thyroid Carcinoma: An International Multicenter Study

Abstract: The 5.6% prevalence of bilateral foci in sMTC suggests that total thyroidectomy should remain the standard of care for initial surgery, as less complete thyroid surgery may fail to address fully the primary site of disease. Whether ipsilateral tumor focality should be an independent factor determining the need for completion thyroidectomy when sMTC is diagnosed after hemithyroidectomy remains to be determined.

“…The diagnosis of MTC prior to surgery is important. MTC can be multifocal or bilateral, 34,35 and metastasis can occur early to locoregional lymph nodes 21,36 . As such, TT with central nodal dissection is required for patients with MTC 29 .…”

“…MTC‐only syndrome is very similar to sporadic MTC, as it arises at an older age, i.e. in the fifth or sixth decade, does not involve other endocrine organs, and is associated with a more favourable prognosis …”

“…(7), in the present study, we noted no direct clinical benefit to extending the RET germline analysis beyond the hot-spot regions in patients with sporadic MTC with multifocality (22%) since the analysis did not identify any mutations in other exons. Therefore, these cases were true sporadic cases of MTC with multifocal tumors, which had been observed in a recent study (34).…”
