Multidisciplinary Team Utilizing Pharmacists in Multimodal, Bundled Care Reduce Chronic Obstructive Pulmonary Disease Hospital Readmission Rates

Abstract: Background: Chronic obstructive pulmonary disease (COPD) is a major contributor of morbidity and mortality in the United States resulting in high hospitalization and readmission rates. For health systems, identifying an effective strategy to reduce COPD readmissions has remained difficult. Multiple COPD care bundles have been developed with varying degrees of success. Bundles that were multidisciplinary and included pharmacists were successful in reducing readmissions. Objective: To describe and assess a multi… Show more

“…Nine of the MDT studies present better disease-specific clinical outcomes for different conditions (67-69, 73, 75, 80, 81, 87, 92). These findings support the positive effects of MDTs, which is in line with the findings of Western MDT studies (106,107).…”

“…Hospital readmissions are a worldwide, major health-care burden, with a negative impact on patient health, public health, and healthcare systems' associated national financial burden. Previous publications describe the heavy toll of hospital (non-surgical) re-admissions while the same researchers and others attempted various strategies for either predicting the risk for readmission and proactively reducing it (albeit with conflicting results) [5][6][7][8][9][10].…”

“…10 Existing studies on collaborative COPD care models focused mainly on clinical pharmacists. 6,51 Considering the importance of continuity of care in improving COPD patient outcomes (including a reduction in all-cause mortality 52 ), a collaborative care model with guided practice protocols should be developed to integrate community pharmacists into the ongoing therapeutic support for COPD patients and to promote the understanding of and respect for pharmacists' potential to contribute, and most importantly, to guide good practice among pharmacists. 5,[53][54][55] To expand and consolidate the role of community pharmacists in managing COPD, it is important to demonstrate the added value of pharmacists' interventions on the overall disease management and the clinical outcome of the patients.…”

“…One of the key reasons for this is that patients face many medication-related problems, including low medication adherence rates, 3,4 inability to use inhaler devices correctly, 5 and failure to take medication properly. 6 The medicationrelated issues that COPD patients face and the consistent care that they need to manage their chronic conditions highlight the importance of the professional role of pharmacists. 7 The responsibility of the pharmacist is no longer simple drug-adjustment work, but has been expanded to become patient-centric diversified health management.…”
