Multidisciplinary strategies for the management of heart failure patients at high risk for admission

Abstract: Multidisciplinary strategies for the management of patients with HF reduce HF hospitalizations. Those programs that involve specialized follow-up by a multidisciplinary team also reduce mortality and all-cause hospitalizations.
