Multicolor FISH (UroVysion) Facilitates Follow-up of Patients With High-Grade Urothelial Carcinoma of the Bladder

Abstract: The aim of the present prospective study was to assess the diagnostic benefit of UroVysion (Vysis-Abbott Laboratories, Downers Grove, IL) in the follow-up of patients with a history of high-grade non-muscle-invasive urothelial carcinoma of the bladder (NMIBC). An unselected cohort of 25 patients with a history of high-grade NMIBC was prospectively followed up by office-based cystoscopy, cytology, and UroVysion in 210 events. The sensitivity and specificity for standard combined cystoscopy and cytology were 78%… Show more

“…A positive FISH result was superior to cytology as a marker of relapse (hazard ratio = 6.2 and 1.4, respectively) when positive G3 cytology was excluded from the analysis. Two other recent studies confirmed that a positive FISH result in BCG-treated patients with a history of high-grade non-muscle-invasive UC predicts relapse independently of cytology and cystoscopy results [43,45]. …”

“…Cytology after intravesical bacillus Calmette-Guerin (BCG) treatment of high-grade non-muscle invasive UC is notoriously difficult, since the reactive changes can appear most worrisome [25,42,43]. We found that patients with a positive post-BCG cytology or FISH result had a 3.2- and 3.8-times higher risk of recurrence, respectively, when compared to the patients with negative results [25].…”

“…Following this, a number of studies have shown promising results for FISH technology (18,19). In a comparative study of FISH with the diagnostic markers, BTA Stat, hemoglobin strips and telomerase, FISH exhibited the highest sensitivity (81% compared with 78, 74 and 46% for BTA Stat , hemoglobin strips and telomerase, respectively) and the highest specificity (96%) (17).…”

“…Second, the presence of histologically confirmed CIS accompanying invasive carcinoma represents an adverse prognostic parameter by itself 34. This prognostic information obtained by Ki67 staining of bladder biopsies represents an advantage compared with UroVysion FISH, a useful non-invasive adjunct for the diagnostic work-up of flat urothelial lesions with a high positive rate of 94% in CIS 35. However, UroVysion FISH requires equipment with a fluorescence microscope which may not be available in every pathology lab and, therefore, should best be viewed as a useful method for complementing rather than replacing bladder biopsy.…”
