Multicenter randomized trial of a comprehensive hospital discharge and outpatient heart failure management program

Abstract: Aims: Disease management programs can reduce hospitalizations in high-risk heart failure (HF) patients, but generalizability to the population hospitalized for HF remains to be proven.
