Mucinous Proliferations of the Uterine Corpus: Comprehensive Appraisal of an Evolving Spectrum of Neoplasms

Abstract: A variety of endometrial lesions may contain mucinous cells. Herein, the author reviews the literature on the classification and clinicopathologic significance of uterine corpus proliferations with a significant mucinous component, assesses the 2020 World Health Organization classification of such lesions, and presents a diagnostic framework. The key epithelial mucinous lesions include mucinous metaplasia, atypical mucinous glandular proliferation and mucinous carcinoma. Each of these categories are classifiab… Show more

“…These related and overlapping entities have been comprehensively covered in a review article by Fadare. 45 The morphological and immunohistochemical overlap between these M€ ullerian lesions and gastric-type lesions is probably under-recognised. M€ ullerian-type mucinous lesions may focally exhibit pale eosinophilic cytoplasm and decreased expression of ER/PR, 58 which underscores the importance of identifying more specific features such as goblet cells and neuroendocrine cells.…”

“…In the 2020 WHO Classification, 13 ‘gastric (gastrointestinal)‐type mucinous adenocarcinoma’ is recognised as a distinct histological type of endometrial carcinoma, while cases which would have previously been classified as mucinous carcinoma (based on the presence of abundant intracytoplasmic mucin in more than half of the tumour) are categorised as endometrioid carcinoma with mucinous differentiation if gastric/gastrointestinal differentiation is absent 45 . Benign or atypical gastric‐type mucinous lesions of the endometrium are not incorporated into the 2020 WHO Classification.…”
