Mucinous cystic tumor of the gallbladder is an extremely rare benign tumor, with potential for malignant degeneration. Mucinous cystic tumors of the cystic duct are divided into mucinous cystadenoma and mucinous cystadenocarcinoma. Currently, cystadenoma is generally considered to be a precancerous lesion of cystadenocarcinoma. At present, there are few cases reported worldwide, and there are no relevant guidelines for diagnosis and treatment of this disease. This article presents the collected clinical data of a patient with mucinous cystic tumor of the gallbladder who was admitted to the First Affiliated Hospital of Hunan Normal University, with the characteristics of the disease summarized in combination with a focused literature review.

The patient was a 70-year-old woman in whom examination revealed a high level of carbohydrate antigen 19-9. Abdominal ultrasonography and computed tomography (CT) revealed a multilocular cystic lesion compressing the gallbladder. CT indicated the presence of a multilocular cystic tumor (67 × 68 × 72 mm) in contact with the right hepatic lobe. Intraoperative findings indicated that the cyst diameter was 8.5 × 6.0 cm, and the cyst was continuous with the gallbladder. The gallbladder was resected along with the cyst. The cyst was multilocular and originated from the cystic duct and gallbladder wall. The cyst wall contained cuboidal to columnar mucin-producing epithelial cells and ovarian-like stroma (OS). The final diagnosis was mucinous cystic neoplasm (MCN) of the gallbladder with low-grade dysplasia. In the 2010 WHO classification of tumors of the digestive system, MCN have been newly defined as a type of hepatobiliary tract epithelial neoplasms. MCN of the gallbladder with OS is extremely rare. Only three cases have been published in the literature. The presence of OS is necessary for diagnosis of MCN.

Imaging Findings of Mucinous Cystadenoma of Cystic Duct With Biliary Obstruction: A Case Report

Mucinous cystadenoma of the gallbladder account for 0.02% of the total number of cases in the hepatobiliary system. By searching the literature from 1900 to 2020, we found 16 cases of mucinous cystadenoma of the gallbladder. The first report of mucinous cystadenoma of the gallbladder was published in 1901 by Bishop. There are only 3 cases of bile duct obstruction caused by gallbladder mucinous cystadenoma extending into bile duct [1]. Meanwhile, the previous literature reports appear lack of complete imaging data. We report a rare case of mucinous cystadenoma of the cystic duct extending into the common bile duct causing biliary obstruction. It is the first case that describes the imaging findings of the disease in detail.
