Mucinous Cystadenocarcinoma of the Appendix Invading the Ascending Colon with Fistula Formation: Report of a Case

Abstract: Based on colonoscopy findings, we made a preoperative diagnosis of primary mucinous cystadenocarcinoma of the appendix with features of a submucosal tumor (SMT) in the ascending colon. A 59-year-old woman who presented with right lower quadrant abdominal pain underwent colonoscopy, which revealed an SMT with three nodules covered with mucus in the ascending colon. Examination of colonoscopic biopsy specimens indicated "very" well-differentiated adenocarcinoma with mucus lakes. Abdominal computed tomography sho… Show more

“…There was no case in which an adenoma existed on the other orifice. There was one report in which a mucinous cystadenocarcinoma of the appendix invaded the ascending colon with fistula formation [10]. It could be argued that our case did not represent an anomaly of the appendix, but rather a fistula caused by an appendiceal neoplasm.…”

“…A number of case reports have been published within the past several decades describing various types of fistulous tracts. The most common organs involved include skin,8-12 bladder,13-20 vagina,21,22 and colon 23,24. In the majority of the cases reviewed, patients were symptomatic from the fistula formation, and this eventually led to the diagnosis of appendiceal mucinous neoplasm upon further investigation via imaging, endoscopy, or surgical exploration.…”

“…Mucinous cystadenocarcinomas demonstrate high-grade cellular dysplasia and frequently invade the stroma beyond the muscularis mucosa [2]. Mucoceles of the colon are uncommon [1, 3].…”

“…Mucoceles of the colon are uncommon [1, 3]. Almost all such tumors occur at the appendix [2, 4, 5, 6]. To our knowledge, this is the first report of mucinous cystadenocarcinoma on the cecum wall during pregnancy.…”
