Mucinous cholangiocarcinoma is extremely rare and its clinicopathological features remain unclear. The present study aimed to analyze published data on mucinous cholangiocarcinoma. Medical databases were searched from 1980 to 2016, and clinicopathological data for 16 mucinous cholangiocarcinoma patients were obtained. Characteristic imaging findings, including hypovascular tumor with peripheral enhancement on computed tomography and angiography, extremely high intensity on T2‐weighted magnetic resonance images, intratumoral calcification and luminal communication between the tumor and bile duct on cholangiography, were noted. Mucinous cholangiocarcinoma was correctly diagnosed in one patient only, with some patients diagnosed as low‐malignant biliary cystic tumors preoperatively. Five cases were followed up after the first medical examination, and three of these were initially diagnosed as biliary cystadenoma or intraductal papillary neoplasm of the bile duct. All five tumors showed marked enlargement within 4 months of follow up. Macroscopically, the resected tumors were non‐cystic/solid in seven cases, and cystic in seven. Tumor diameter ranged from 5 cm to 22 cm, and mucoid cut surface, lobulation, lack of capsule and papillary growth were observed. Microscopically, co‐existing intraductal papillary neoplasm of the bile duct was noted in three of five patients with available data. Nine of 10 cases in whom the pathological stage was reported had advanced disease with lymph node and/or distant metastasis, and 5‐year survival was achieved in one microinvasive case only. Overall 1‐ and 3‐year survival rates were 60.1% and 40.1%, respectively. The possibility of mucinous cholangiocarcinoma should be considered when biliary cystic tumors are detected on imaging modalities, despite the rarity of this tumor.

Abstract. Twenty-four adenocarcinomas of the hepatobiliary system were found among I10 primary hepatic neoplasms; 22 of these were intrahepatic, one involved the extrahepatic bile duct and one the gall bladder. Histologically, 10 intrahepatic neoplasms were classified as cholangiocarcinoma, and 12 as bile duct cystadenocarcinoma. The former were characterized by tubular structures lined by anaplastic cuboidal or columnar cells with diffuse fibrous stroma, and the latter by multiple cystic structures with papillary and solid areas. Half the bile duct cystadenocarcinomas showed benign cysts and transition to adenocarcinoma. This was seen in only one case of cholangiocarcinoma. The extrahepatic bile duct carcinoma was characterized by tubular structures lined with flattened cuboidal cells with scant stroma. The gall bladder adenocarcinoma contained many acinar structures lined with columnar or cuboidal cells and separated by thin stroma. Distant metastasis was found in 87.5% (21 of 24) of the bile duct adenocarcinomas; the lymph nodes, lungs and peritoneum were the common sites of metastasis.The biliary system can be divided anatomically into three segments: the intrahepatic and extrahepatic bile ducts, and the gall bladder. Carcinomas of all three segments have been described in man [I, 7, 8, 13 We analyze 24 biliary system neoplasms, and describe their clinical, gross morphologic and histologic characteristics. Materials and MethodsThe 110 primary hepatic neoplasms found in 12,245 necropsies of dogs done at The Animal Medical Center were studied as described in a previous report [ 151. Of these neoplasms, 24 originated from the biliary system: 22 from the intrahepatic bile ducts, one from an extrahepatic bile duct and one from the gall bladder.Formalin-futed tissues were processed routinely, and sections 4 to 6 pm thick were stained with hematoxylin and eosin (HE), mucicarmine, and periodic acid-Schiff (PAS). ResultsThe frequency of bile duct carcinoma in this study was 22% of the primary hepatic neoplasms and 0.2% of the necropsy population. The majority of bile duct carcinomas occurred in dogs over 10 years of age (63%; 15 of 24) and in females (58%; 14 of 24). The neoplasms were found in eight different breeds, and no breed predisposition was found. Anorexia, lethargy, ascites and dyspnea were the common clinical signs. Thirteen percent (2 of 15) were icteric; one of these was the dog with extrahepatic bile duct carcinoma. One dog had myelopathy caused by metastasis, one had seizures, and one had vomiting. Leukocytosis and increased serum alkaline phosphatase were recorded in three of four dogs.Gross morphologic descriptions adequate to classify the intrahepatic bile duct carcinomas into massive, nodular or diffuse lesions were available for 18 dogs. The clinicopathologic features of the intraductal growth (IG) type of peripheral cholangiocarcinoma (PCC) were examined retrospectively. Out of 112 patients who underwent surgery for PCC at Seoul National University Hospital, Seoul, Korea, between January 1980 and December 1997, 16 were classified as having the IG type. Thirteen were men and 3 were women. Their ages ranged from 38 to 73 years with a mean age of 55.9 years. Abdominal pain was the most common symptom and jaundice was found in 18.8%. Five patients had associated clonorchiasis and 6 patients experienced hepatolithiasis. Tumor sizes ranged from 0.5 cm to 14.0 cm with a mean of 4.3 cm. The tumor was located in the right lobe in 7 cases and the left lobe in 9 cases. Thirteen patients underwent major hepatic resection, and 3 underwent minor resection, involving a subsegmentectomy. Mucin was found in the bile in 4 cases. Pathology showed papillary adenocarcinoma with a background of adenomatous hyperplasia, and the absence of lymph node metastasis in all cases. The tumors were confined to the mucosa in 5 cases. Out of the 16 hepatic resections, 1 was palliative due to a positive margin. In 2 of the patients who underwent minor resections, recurrences developed, and 1 of these died 56 months after resection. Out of the 16 patients, 15 remain alive, ranging from 1 to 13 years, postoperatively. In conclusion, the IG type of PCC should be distinguished from other types of PCC because a favorable prognosis can be expected after complete surgical resection. (HEPATOLOGY 2000;31:12-17.)Intrahepatic or peripheral cholangiocarcinoma (PCC), a primary adenocarcinoma of the liver originating from the intrahepatic bile ducts (IHD), is the second most common primary hepatic malignancy, next to hepatocellular carcinoma. 1 Because the lesion is advanced by the time of diagnosis, 2 the possibility of surgical resection is limited and the prognosis is generally poor. [3][4][5][6][7][8][9] In 1992, Yamamoto et al. 10 classified PCC into 2 types, mass-forming (MF) and periductal-infiltrating (PI), and showed their different clinical behaviors. The MF type creates an apparent lesion in the liver, with frequent remnant hepatic recurrence after resection. The PI type causes a stricture or an obstruction of the IHD without forming a definite mass in the liver. It spreads along the Glisson' s capsule and metastasizes to the hilar lymph nodes.
