EinleitungGroßschadensereignisse mit einem "Massenanfall von Verletzten oder Erkrankten" (MANV) stellen eine außerordentliche Herausforderung in der Notfallmedizin dar. Unvorhersehbar in Art und Ausmaß bedingen sie enorme Anforderungen an lokale, regionale und über-regionale Rettungsdienststrukturen. Auslöser können u. a. Verkehrsunfälle mit Bussen und Bahnen, Flugzeugabstürze, Großbrände, Explosionen, der Einsturz von Gebäuden, Naturkatastrophen oder Terroranschläge sein. Wenn der tatsächliche Bedarf die Kapazitäten des lokalen Rettungsdienstes überfordert, ist nur durch eine effektive und strukturierte Zusammenarbeit der Einsatzkräfte vor Ort die Rettung und Wiederherstellung der Gesundheit möglichst vieler Patienten möglich. Zentrale Bestandteile der Arbeit bei Großschadens-lagen sind die Sichtung (Triage) und die Bildung einer lokalen Infrastruktur zur frühestmöglichen Sicherstellung einer adäquaten medizinischen Versorgung mög-lichst aller Betroffenen. Dabei soll immer die Sicherheit aller Rettungskräfte und Helfer gewahrt bleiben. Die Bewältigung größerer Notfallereignisse setzt eine intensive Auseinandersetzung der Beteiligten mit grundlegenden Strategien und einsatztaktischen Über-legungen bereits im Vorfeld voraus. Dabei muss im Ernstfall "mit einer Sprache gesprochen werden." In diesem Zusammenhang ist es für den notfallmedizinisch Tätigen wichtig, einen Überblick darüber zu gewinnen, auf welche Einsatzkräfte im Großschadensfall zurückgegriffen werden kann. In vielen Regionen Deutschlands gibt es bereits von entsprechenden Kommissionen erarbeitete MANV-Konzepte, die der lokalen und überregionalen Vorhaltung von Rettungsmitteln Rechnung tragen. Oberstes Ziel aller Hilfeleistungen bei Großschadens-lagen ist die optimale Patientenversorgung sowie die Rettung und Wiederherstellung der Gesundheit mög-lichst vieler Verletzter bzw. Erkrankter. Hierbei tritt die medizinische Individualversorgung vorübergehend in den Hintergrund nach dem Grundsatz "das Beste tun für die meisten Patienten." Angestrebt wird eine frü-hestmögliche Rückkehr zur adäquaten individualmedizinischen Versorgung [1]. Definitionen Grundsätzliches Merkmal von Großschadensereignis-sen ist die akute Verknappung von Ressourcen. Die Anzahl von Verletzten bzw. Erkrankten übersteigt plötzlich und unerwartet die vorhandenen oder innerhalb kürzester Zeit verfügbaren Helfer. Dies ist im Einzelfall eng an geografische und einsatztaktische Umstände geknüpft. In ländlichen Gebieten wird die lokale rettungsdienstliche Infrastruktur schneller an ihre Grenzen stoßen als in Großstädten oder Ballungszentren. This paper describes a methodology of exercise monitoring using qualitative and quantitative tools. These tools document the exercise time flow, gauge appropriate measures and decisions and collect feedback of exercise participants. These three elements provide a basis to assess the quality of the undertaken measures. The documentation of the operational performance helps to enhance emergence preparedness. This research has great potential to lead to operational insights at the micro-level.
