MRT der Prostata: Empfehlungen zur Vorbereitung und Durchführung

Abstract: The Working Group Uroradiology and Urogenital Diagnosis of the German Roentgen Society has developed uniform recommendations for the preparation and implementation of prostate MRI. In the first part detailed recommendations are given in tabular form regarding 1. anamnestic data before prostate MRI, 2. termination of examinations and preparation of examinations, 3. examination protocol and 4. MRI-guided in-bore biopsy. In the second part, the recommendations are discussed in detail and relevant background infor… Show more

“…The joint recommendations of the American College of Radiology (ACR), the European Society of Urogenital Radiology (ESUR) and the AdmeTech Foundation regarding performance of MP-MRI of the prostate as well as standardized findings (PI-RADS v2) are an important step towards an international standard which can strengthen the trust of the referring physicians in the method [12]. In the German-speaking realm, one step in this direction includes, among other things, current standardized implementation and protocol recommendations for prostate MRI [25]. Individual studies which continue to promote the use of their own Likert scales suggest that uniform, standardized diagnosis must be further developed [26].…”

“…Additionally, systematic biopsy alone fails to detect up to 20 % of significant carcinomas [7]. Today, mpMRI, consisting of T2w, DW, and dynamic contrast-enhanced (DCE) imaging, has shown substantial value in the staging, detection, localization, characterization, risk stratification, surveillance, and assessment of suspected recurrence of PCa [6]. Data from systematic reviews and diagnostic studies indicate that a combination of MRI-assisted, targeted plus systematic biopsy achieves much better detection rates than the respective methods alone [2,8].…”

“…This demographic development must be taken into account in the diagnosis, treatment, and follow-up care of PCa. Conventional diffusion-weighted imaging (DWI) was shown to add incremental value to T2-weighted (T2w) magnetic resonance imaging (MRI) in the detection and localization of PCa, making it a key component of the prostate multiparametric MRI (mpMRI) exam [4][5][6].…”

“…Axiale a und koronale b T2-gewichtete Bilder: signalgemindertes Prostatakarzinomsuspektes Areal links in der Transitionszone mediobasal (Pfeile). Fokal abgesenkter scheinbarer Diffusionskoeffizient (ADC) c.
