MRI With Gadolinium as a Measure of Blood-Labyrinth Barrier Integrity in Patients With Inner Ear Symptoms: A Scoping Review

Abstract: Objective: Capillaries within the inner ear form a semi-permeable barrier called the blood-labyrinth barrier that is less permeable than capillary barriers elsewhere within the human body. Dysfunction of the blood-labyrinth barrier has been proposed as a mechanism for several audio-vestibular disorders. There has been interest in using magnetic resonance imaging (MRI) with intravenous gadolinium-based contrast agents (GBCA) as a marker for the integrity of the blood labyrinth barrier in research and clinical s… Show more

“…Affirmative findings for ISSNHL on 3D FLAIR have been reported subsequently ( 13 ); however, methodological differences in terms of the methods of MRI assessment, contrast-medium administration protocol, and assessment criteria for hearing improvement might have led to inconsistencies in the results ( 14 ). Song et al ( 15 ) have also reported that correlations between abnormal enhancement and clinical prognosis varied among investigations.…”

“…MRI studies of patients with CAPS or non-syndromic autoinflammatory inner ear disease have also identified cochlear enhancement (7)(8)(9). Intravenously injected contrast material can reach the inner ear via small vessels that supply the labyrinth and cochlea (23). Capillary networks present in the stria vascularis and spiral ligament form the blood-labyrinth barrier, composed of endothelial cells with tight-junctions surrounded by pericytes and resident macrophages (24).…”

“…Исследования с использованием МРТ с контрастированием на здоровых добровольцах и животных показали по- степенное повышение интенсивности сигнала от внутреннего уха, ограниченное перилимфой. Пик накопления контрастного препарата у человека составляет 4 ч [21]. Показано, что 3D-FLAIR является более чувствительной МР-последовательностью в сравнении со стандартными [22,23].…”
