MRI signal changes of the bone marrow in HIV-infected patients with lipodystrophy: correlation with clinical parameters

Abstract: A serous-like MRI pattern is observed in the peripheral skeletons of HIV-infected patients with lipodystrophy, which correlates with peripheral lipoatrophy, and should not be misdiagnosed as malignant or infectious diseases. Although the MR lesions did not improve after switching the treatment, there was evidence of lipoatrophy recovery.

“…This finding is confirmed by the PDFF map that shows almost no fat present in this region (hematopoietic marrow in the spine typically contains at least 46% to 66% fat). A high prevalence of serous‐like patterns in individuals with HIV has been previously reported . Therefore, these findings emphasize the need for high‐resolution fat fraction mapping.…”

“…A high prevalence of serous-like patterns in individuals with HIV has been previously reported. 168 Therefore, these findings emphasize the need for highresolution fat fraction mapping. Upper rows are a high-resolution trabecular bone image (left column), an R 2 * map (middle column), and a local fieldmap after removal of background fields (right column) of the distal tibia in a healthy subject.…”

“…Other conditions include infiltrative malignancy treated by chemo-radiation/bone marrow transplant, Paget's disease in acute-subacute phases, and serous marrow lesions observed in HIV-infected patients having lipodystrophy syndrome treated with highly active antiretroviral therapy. [575859] These conditions can be highly heterogeneous and difficult to distinguish from active malignancy. However, there are usually small islands of microscopic fatty marrow in successfully treated lesions.…”

“…During brain MRI reading, attention should also be paid to the cranial bone and extracranial lesions, particularly those involving the bone marrow, parotid glands, lymphoid tissue, and skin. HIV causes chronic inflammation and hyperplasia of the bone marrow [ 104 106 ], both of which could result in a decreased signal on T1-weighted images, making the bone marrow appear “gray” [ 107 ]. In addition, HIV and ART both increase the risk of osteoporosis [ 108 ].…”
