MRI criteria for the diagnosis of multiple sclerosis: MAGNIMS consensus guidelines

Abstract: Summary In patients presenting with a clinically isolated syndrome (CIS), magnetic resonance imaging (MRI) can support and substitute clinical information for multiple sclerosis (MS) diagnosis demonstrating disease dissemination in space (DIS) and time (DIT) and helping to rule out other conditions that can mimic MS. From their inclusion in the diagnostic work-up for MS in 2001, several modifications of MRI diagnostic criteria have been proposed, in the attempt to simplify lesion-count models for demonstrating… Show more

“…However, in line with the recommendations of the recently published MAGNIMS consensus guidelines,21 we suggest that spinal cord MRI should be included in the diagnostic workup if the presenting symptoms indicate spinal cord involvement. Furthermore, spinal MRI can be helpful in the differential diagnosis when brain imaging is equivocal or inconclusive and when brain MRI findings suggest radiologically isolated syndrome (RIS) 21, 34…”

“…The MAGNIMS collaboration has recently published suggestions for adjustments34 to the current McDonald criteria for the diagnosis of MS, last revised in 2010 5. Some important points in the suggested adjustments are the recommendations to:…”

“…Specifically, the central vein sign alone or in combination with available MS diagnostic MRI criteria7, 28 improves the diagnostic accuracy and specificity, without lowering the sensitivity, of MS diagnosis. Assessments were performed on standard clinical magnetic field strength systems, and results at 3T and the more widely available 1.5T were indistinguishable.…”

“…Of note, the 2010 revised criteria of McDonald (Polman et al., 2011) and MAGNISM criteria of 2016 (Filippi et al., 2016) define that a second attack can be diagnosed if there is a new T2 lesion or if there is new enhancing lesion.…”
