MRI and Ultrasound Imaging of the Shoulder Using Positional Maneuvers

Abstract: Positional MRI and ultrasound are highly useful in evaluation of the shoulder. Knowledge of the normal appearance of anatomic structures and pathologic changes in nontraditional imaging planes is necessary to avoid pitfalls in interpretation.

“…MR arthrography is highly effective in the detection of labroligamentous lesions with sensitivities ranging from 88% to 96% [27,[49][50][51] and specificities ranging from 86% to 96% [40,50,51]. Provocative positioning maneuvers, including abduction external rotation, are important in improving detection of nondisplaced anteroinferior labral tears [52][53][54] and have been shown to be helpful in the evaluation of multidirectional instability [55]. Flexion-adduction internal rotation may improve assessment of the posterior labrum and capsule [52].…”

“…Both shoulders were evaluated in each individual scan and results were noted for both dominant and non-dominant arms. My Lab, 7 eSaote ultrasound Machine with, [8][9][10][11][12][13][14][15] MHz linear array probe was used. Ultrasound was done according to the decorum recommended by the European Society of Musculoskeletal Radiology.…”

“…The subscapularis tendon thickness was recorded just medial to the insertion site. The modified crass position was used to measure the thickness of supraspinatus tendon 12 . The supraspinatus tendon was visualized on the coronal view at the sulcus between greater tuberosity and articular cartilage in the modified crass position.…”
