“…Lactate is an end-product of oxidative metabolism and is therefore a potentially interesting biomarker for a metabolism-associated illness such as ME/CFS. Lactate levels in healthy brain tissue are so low as to be almost undetectable by conventional MRS at 1.5T or 3T magnet strength, but when measured in ventricular cerebrospinal fluid, elevated lactate is associated with neuroinflammation (33, 8792).…”

“…This in line with previous work stated that, lactate is only found in pathologic states and acute stroke due to anaerobic glycolysis performed by the brain in ischemia or due to macrophage activation after membrane break down, as macrophages uses lactate for their energy supply [12] . Karatag et [13] evaluated the peak of LL in all cases and used it if present with Cho/Cr > 2.2 to differentiate high-from low grade neoplastic lesions with a sensitivity of 100% [82.2-100] and a specificity of 100% Kaddah et al [14] demonstrated that, the characteristic metabolite change of encephalitis was significant reduction of NAA and NAA/Cr ratio. This reduction was significant in the chronic phase of the disease, and gradual recovery was observed within 1 year.…”

“…MRI showed an isolated ovoid lesion at the SCC. The MR metabolites spectrum did not support the ischemic hypothesis, as in ischemia the NAA peak is usually reduced with elevated peaks of Lac/Lip, nor the neoplastic or acute demyelinizing hypotheses, as in tumors and multiple sclerosis acute plaques the Cho peak is usually elevated, with Cho/NAA ratio >1 [23][24][25].…”

“…90 Diagnosis is confirmed by the disappearance of the lesion within a few weeks [8]. Magnetic Resonance Spectroscopy (MRS) detects brain metabolites alterations, providing further information about the nature and of the lesions [23][24][25]. There are few previous studies of MRS of SCC transient lesions which 95 demonstrated a normal spectrum of the main brain metabolites such as N-acetylaspartate (NAA), choline (Cho), © and creatine (Cr), but presence of peaks of myoinositol (MI) and lactate (Lac), not detectable in normal brain parenchyma, suggesting hyperosmolar state and increased anaerobic glycolysis in the 100 splenium without irreversible neuroaxonal damage [23][24][25].…”
