MR imaging surface display of the cerebral cortex in children

Abstract: The purpose of this study was to introduce the use of three-dimensional (3D) surface MR imaging display for clinical use. Surface display was created using images acquired with T1 magnetization prepared rapid acquisition gradient echo (MPRAGE) in 24 cases of migrational defects, cortical dysplasia and prenatal asphyxia. Schizencephaly and cortical dysplasia were pathologically confirmed. The precise configurations of cortical abnormalities, and their relation to the adjacent gyri and sulci were demonstrated. “…Although some texts report that ulegyria is usually unilateral, seven of our eight patients had bilateral involvement [11,16]. Interictal and ictal EEG recordings in our cases showed predominantly temporal lobe or poorly localized epileptiform activity, while seizure semiology after the aura was consistent with frontal and temporal lobe activation.…”

“…Volume rendering provides a 3D view of the surface and therefore has a potential in assisting the surgeon in preoperative planning (9). Volume rendering is potentially a useful adjunct to standard image review when displaying the abnormal gyri and sulci associated with cortical malformations, and may help in defining the extent of the abnormality.…”

“…Digital Subtraction Angiography (DSA) was the gold standard for intracranial vascular imaging until a few years ago, but it is an invasive procedure with a non-negligible rate of complications (von Lanz and Wachsmuth, 1985;Laub, 1994;West et al, 1997). The method of choice for presenting and evaluating cerebral veins has been described in the literature (Huber, 1982;Lee et al, 1997). The bridging veins and the large cortical anastomotic veins examined in this study can be visualized with CT angiography in 76-97% of cases.…”
