MR Imaging of the Spine and Sacroiliac Joints for Spondyloarthritis: Influence on Clinical Diagnostic Confidence and Patient Management

Abstract: MR imaging of the spine and sacroiliac joints significantly influences the diagnostic confidence of rheumatologists regarding clinical features and overall diagnoses of axial spondyloarthritis, and consequently significantly affects treatment plans.

“…27 If inflammatory arthritides are considered, MR imaging (MRI) would be the favored initial imaging modality because of its high sensitivity and ability to detect concurrent noninflammatory causes of pain. 28,29 When degenerative disease is absent, posterior hip pain originating from the spine is usually secondary to a herniated disc, in which case MRI can be used to confirm the diagnosis. 30 Proximal hamstring tendinopathy is sometimes referred to as hamstring syndrome and can produce symptoms mimicking sciatica in addition to focal posterior hip pain.…”

“…MRI of the sacroiliac joints (SIJs) is the preferred imaging examination in patients with back pain clinically suspected to have axial spondyloarthritis (SpA) when pelvic radiographs show normal or equivocal findings 1. Global assessment of SIJ MRI is the common diagnostic approach with simultaneous evaluation of T1-weighted spin echo (T1SE) and short τ inversion recovery (STIR) sequences globally, not restricted to a single lesion type.…”

“…Whole-body MRI allows global recognition and assessment not only of marrow edema adjacent to affected joints, but significantly in the early stages of the disease can identify enthesitis before bone marrow changes have become established, allowing early institution of aggressive treatments, which improve patient outcomes. 81 In established ankylosing spondylitis, early stages are characterized by inflammatory marrow edema adjacent to the sacroiliac joints and peripheral entheses. Similar changes are evident at the vertebral corners; however, an almost identical appearance can be seen in the spine in degenerative disk disease and caution is required with interpretation.…”
