Mp22-07 outcomes of Dorsal Onlay Buccal Mucosal Graft Urethroplasty in Patients With Post-Prostatectomy, Post-Radiation Anastomotic Contracture

Abstract: INTRODUCTION AND OBJECTIVE: Treatment of patients with vesicourethral anastomotic contracture (VUAS) /membranous stenosis (Sp) following a combination of prostatectomy and radiation treatment for prostate cancer (CaP) are understudied. We evaluate feasibility of dorsal onlay buccal mucosal graft urethroplasty (D-BMGU) and evaluate patency and continence outcomes in patients with a history of prior radical prostatectomy and radiation therapy.METHODS: Retrospective multi-institutional review of patients with pos… Show more

“…We appreciate the observations and acknowledgment of our work 1 by Drs Warner and Johnsen. Their editorials provide valuable insight into the evolving landscape of urological reconstructive surgery, especially concerning the use of grafting in radiated fields—a practice once deemed contraindicated.…”

“…In this multi-institutional study, the authors present their technique and outcomes in managing patients with nonobliterative posterior urethral stenosis following both radical prostatectomy and radiation therapy for prostate cancer. 1 Utilizing a dorsal onlay buccal grafting approach, they were able to achieve success in 38/45 patients (84%), with no identifiable incidences of de novo stress urinary incontinence at a median follow-up of 21 months.…”

“…Taken together, while this is a technically challenging operation, it should be accessible to most who are already performing posterior urethroplasty and should be considered a viable alternative to more traditional transecting approaches. Niels V. Johnsen 1…”

“…We commend the authors on their report of outcomes for dorsal onlay buccal mucosa urethroplasty in this fragile patient population. 3 The proposed technique confers an optimistic 85% success rate where complications were infrequent. This is inconsistent with previously reported surgical reconstruction outcomes in this patient population using a combined robotic and perineal approach; the results are humbling (including our own) and did not reflect the outcomes of this multi-institutional series.…”
