Motorcycle accidents are becoming a public health problem in developing world. The objective of this paper was to assess the factors related to morbidity and mortality among victims of motorcycle accidents in a trauma center. An analysis of 9,734 medical records of patients hospitalized for external causes at the Regional Emergency and Trauma Hospital of Campina Grande, Paraiba, Brazil, from January to December 2009 was done. The cases of accidents with motorcycles accounted for 20.8% of all hospital care, mostly affecting men (85.8%) aged 21-29 years old (33.9%). Legs and feet (55.2%) and arms and hands (26.9%) were the most affected parts of the body. The association between the occurrence of motorcycle accident affecting the face (p=0.001), the chest (p<0.001), arms and hands (p=0.004) and legs and feet (p<0.001) was observed. Multiple lesions were present in 40.4% of the cases; and the majority (80.6%) involved fractures. Amputation and functional impairment were observed in 15.3% and 4.3% of the victims, respectively. Vascular lesions in the lower limbs had a 3.5 times higher chance of occurrence among injured motorcyclists. Fatal accidents accounted for 1.3% of the injured victims. In conclusion, motorcycle accidents affect men, predominantly, most of them being young, causing fractures and multiple lesions, mainly in the lower limbs. There is need for greater incentive to actions concerning traffic safety education focusing on motorcyclists, as well as greater control by competent authorities as regards to the use of motorcycles in the country. _________________________________________________________________________________________

Risk factors associated with facial fractures

The aim of the present study was to identify risk factors for facial fractures in patients treated in the emergency department of a hospital. The medical charts of 1121 patients treated in an emergency ward over a three-year period were analyzed. The independent variables were gender, age, place of residence (urban or rural area) and type of accident. The dependent variables were fractured mandible, zygoma, maxilla, nasal bone and more than one fractured facial bone. Statistical analysis was performed using the chi-square test (α < 0.05), univariate and multivariate Poisson distributions and the logistic regression analysis (p < 0.20). Maxillofacial trauma was recorded in 790 charts (70.5%), with 393 (35.1%) charts reporting facial fractures. Motorcycle accidents were found to be the main risk factor for mandibular fractures (PR = 1.576, CI = 1.402-1.772) and simultaneous fractures of more than one facial bone (OR = 4.625, CI = 1.888-11.329) as well as the only risk factor for maxillary bone fractures (OR = 11.032, CI = 5.294-22.989). Fractures of the zygomatic and nasal bones were mainly associated with accidents involving animals (PR = 1.206, CI = 1.104-1.317) and sports (OR = 8.710, CI = 4.006-18.936), respectively. The determinant for the majority of facial fractures was motorcycle accidents, followed by accidents involving animals and sports.

Objective: To assess the pattern of dentofacial injuries in commercial motorcycle accidents among riders and passengers in Cameroon. Methods: This was a hospital based study conducted in 6 out of 10 regional capitals in the months of December 2011 to September 2012. Analyzed information included age, gender, residence, role on the motorcycle (rider or passenger), type, pattern and month of injury, cost, duration and patient's perception about the cost of treatment. Results: A total of 387 patients were studied with majority of the patients being 21-30 years (39.8%), males (63.8%), passengers (57.3%) and urban dwellers (85.8%). Most of the injuries occurred in December (20.7%), January (19.4%) and February (20.2%). Soft tissue injuries were most frequent (91.2%) followed by trauma to the teeth (83.5%), of which 62.3% were tooth loss. Mandibular fracture was commoner than maxillary fracture; (45% versus 25.3%). A total of 44.2% of patients received their treatment as in-patients. The treatment of the dentofacial injuries among 64.3% of the patients lasted for more than a month. A total of 51.9% of the patients spent 100,000 francs ($200) or more for their treatment. More than half (51.4%) of the patients perceived the cost of treatment as expensive. Conclusion: Dentofacial injuries in commercial motorcycle accidents necessitated hospital admission and lengthy treatment time with high attendant cost. Preventing these injuries will serves as a form of poverty reduction as money that will be used by the victim to better their life is not used to correct deformities or treat injuries.
