OCCUPATIONAL APPLICATIONS Many new occupational back-support exoskeletons have been developed in the past few years both as research prototypes and as commercial products. These devices are intended to reduce the risk of lowerback pain and injury for workers in various possible application sectors, including assembly in automotive and aerospace, logistics, construction, healthcare, and agriculture. This article describes the technologies adopted for back-support exoskeletons and discusses their advantages and drawbacks. Such an overview is intended to promote a common understanding and to encourage discussion among different stakeholders such as developers, ergonomics practitioners, customers, and workers. TECHNICAL ABSTRACT Background: The large prevalence and risk of occupational lower-back pain and injury associated with manual material handling activities has raised interest in novel technical solutions. Wearable back-support exoskeletons promise to improve ergonomics by reducing the loading on the lumbar spine. Purpose: Since many new prototypes and products are being developed, this article presents an up-to-date overview of the different technologies. By discussing the corresponding advantages and drawbacks, the objective is to promote awareness and communication among developers, ergonomics practitioners, customers, and factory workers. Methods: The state-of-the-art is presented with a focus on three technological aspects: (i) the actuators generating assistive forces/torques, with a main distinction between passive and active devices; (ii) the structures and physical attachments that transfer those forces/torques to the user, with structures being soft, rigid, or a combination of the two; and (iii) the control strategies employed (i.e., how devices adjust assistive forces/torques to accommodate different activities and parameters). Discussion: The choice of actuation technology may determine the applicability of a device to different scenarios. Passive exoskeletons appear more suitable for tasks requiring relatively light assistance and little dynamic movements. By contrast, heavier and more dynamic tasks will justify the use of more complex active exoskeletons. Most assistive robotic devices are exoskeletons which assist or augment the motion of the limbs and neglect the role of the spinal column in transferring load from the upper body and arms to the legs. In the SPEXOR project we will fill this gap and design a novel spinal exoskeleton to prevent low-back pain in able bodied workers and to support workers with low-back pain in vocational rehabilitation. I. CONTEXT Low-back pain (LBP) is a growing issue in our modern society. From all the diseases studied in the Global Burden of Disease Study from 2010 (Institute for Health Metrics and Evaluation), LBP was ranked number one concerning the years lived with disability in Europe [1]. Regarding the overall burden, LBP was ranked third in Central and Eastern Europe and first in Western Europe [1]. Depending on sex and region, the European point prevalence of LBP was up to 15.6%. Prevalence and burden were found to increase with age and are even expected to further increase within the next years due to the current demographic developments [2]. From several studies reviewing the costs related to LBP in European countries, it has been calculated that the lowest cost per capita in Europe can be found in Belgium with e116 and e209 in Sweden [3]. In this paper, for the purpose of practical use of the power assist suit, the overalls type power assist suit developed in our previous study was improved. The knee pad type power assist suit termed "Cool Vest" was newly developed, and verification experiments were carried out. By wearing the developed assist suit, the muscular load decreased 46% at the maximum in the case of lifting and taking down the 30kg object and decreased 43% at the maximum in the case of transferring the 20kg object by turning around. In addition, air supplying method to the artificial muscle was also improved. An air recycling system and an air generation system were proposed, and evaluation experiments were executed. As results, the number of times which can be used for assisting the transfer work increased from 5 times to 7 times by using these air supplying systems.
